Combined Treatment Protocols for Lyme Borreliosis and Coinfections

Abstract:

Lymeská borrelióza se řadí mezi nejčastější zoonózy přenášené klíšťaty. Je způsobena gramnegativními, mikroaerofilními bakteriemi komplexu Borrelia burgdorferi sensu lato. Kromě borrelií může klíště na člověka přenést i jiné patogeny, nejčastěji to jsou Anaplasma spp., Ehrlichia spp., Bartonella spp., Babesia spp., Francisella tularensis, Coxiella burnetii, Rickettsia spp. nebo virus klíšťové encefalitidy. Lymeská borrelióza a koinfekce jsou léčeny převážně antibiotiky, a to tetracykliny, peniciliny, cefalosporiny a makrolidy. Další možností je kombinovaná léčba, kdy se volí vhodná kombinace antibiotik nebo případně kombinace antibiotik s jinými druhy léčiv. Délka léčby se liší v závislosti na stadiu a závažnosti onemocnění, většinou se jedná o 2–4 týdny. Mezi alternativní druhy léčby se řadí užívání disulfiramu nebo přírodních léčiv. Ke zvládnutí infekcí také napomáhá zdravý životní styl a dobré duševní zdraví pacienta.

Abstract:

Lyme disease is one of the most common tick-borne zoonoses. It is caused by gram-negative, microaerophilic bacteria of the Borrelia burgdorferi sensu lato complex. In addition to Borrelia, ticks can be transmitted to humans by other pathogens, most commonly Anaplasma spp., Ehrlichia spp., Bartonella spp., Babesia spp., Francisella tularensis, Coxiella burnetii, Rickettsia spp. or tick-borne encephalitis virus. Lyme disease and co-infections are mainly treated with antibiotics, namely tetracyclines, penicillins, cephalosporins and macrolides. Another option is combination therapy, in which a suitable combination of antibiotics or possibly a combination of antibiotics with other types of drugs is chosen. The duration of treatment varies depending on the stage and severity of the disease, usually 2–4 weeks. Alternative treatments include the use of disulfiram or natural remedies. A healthy lifestyle and good mental health of the patient also help to manage infections.
