Physiotherapy for a client with rotator cuff tear

Abstract:

Úvod: Tématem bakalářské práce je Fyzioterapie u klienta s rupturou rotátorové manžety. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u pacienta s rupturou rotátorové manžety, využitím vhodných terapeutických metod, postupů a konceptů. Metodika: Tato práce byla zpracována jako případová kazuistika. V přehledu teoretických poznatků byla zpracována výše zmíněná problematika, funkční anatomie, diagnostika a terapie. Hlavní část pak obsahuje studii 30letého pacienta, který hraje poloprofesionálně házenou. Fyzioterapeutické intervence obsahovaly 9 terapeutických jednotek, které proběhly v době 5. týdnů. V rámci intervencí byl odebrán vstupní a výstupní kineziologický rozbor, zároveň proběhlo i kontrolní vyšetření během 5. terapeutické jednotky. Na základě vstupního kineziologického rozboru (antropometrie, goniometrie, funkční svalové testy dle Jandy, vyšetření reflexů, DNS testy, speciální vyšetřovací testy pletence ramenního) byl pak sestaven krátkodobý fyzioterapeutický plán s využitím následujících fyzioterapeutických metod a konceptů: měkké techniky, mobilizace kloubů, postizometrická relaxace, Brügger koncept, DNS, ACT?, centrace ramenního kloubu a další. V rámci 9. fyzioterapeutické jednotky byl odebrán výstupní kineziologický rozbor. Výsledky: Dle porovnání výsledků vstupního a výstupního kineziologického rozboru došlo ke zlepšení vlastností pravého ramenního kloubu v těchto aspektech: odstranění bolestivosti ramenního kloubu (dle VAS 0), zvětšení aktivního rozsahu pohybu ramenního kloubu do ventrální flexe o 10°, abdukce 10°a zevní rotace 15°. U pasivního rozsahu pohybu byly dosaženy stejné hodnoty jako u flexe a abdukce a u zevní rotace se pak rozsah pohybu zvýšil o 20°. Zlepšení také proběhlo u svalové síly, která byla hodnocena na základě kritérií svalových testů dle Jandy. K hlavnímu zlepšení pak došlo u abdukce lopatky, abdukce ramenního kloubu, zevní a vnitřní rotace (vše o jeden stupeň). Sám pacient pak hodnotil výsledky terapie kladně. Závěr: Výsledky poukazují na důležitost řešení problematiky ruptury rotátorové manžety jako komplexního celku, a ne čistě jako otázku glenohumerálního kloubu. Konzervativní fyzioterapie se jeví jako účinná metoda u pacientů s recidivující bolestí a omezeným rozsahem pohybu po ruptuře rotátorové manžety.

Abstract:

Introduction: The topic of the bachelor thesis is Physiotherapy in a client with rotator cuff rupture. The objective: The aim of the thesis is to contribute to the solution of the problem of physiotherapy in a patient with rotator cuff rupture, using appropriate therapeutic methods, procedures and concepts. Methodology: This work was prepared as a case report. In the review of theoretical knowledge the above mentioned problems and their diagnosis, therapy and functional anatomy were treated. The main part then contains a study of a 30-year-old patient who plays semi-professional handball. The physiotherapy interventions included 9 therapeutic units that took place over a period of 5 weeks. Input and output kinesiological analysis was taken during the interventions, and a follow-up examination was also performed during the 5 therapeutic units. On the basis of the initial kinesiological analysis (anthropometry, goniometry, functional muscle tests according to Janda, reflex examination, DNS tests, special examination tests of the brachial plexus), a short-term physiotherapy plan was then drawn up using the following physiotherapy methods and concepts: soft techniques, joint mobilisation, post-isometric relaxation, Brügger concept, DNS, ACT?, Centration of the shoulder joint and others. In the 9 physiotherapy units, an output kinesiological analysis was taken. The results: According to the comparison of the results of the initial and output kinesiological analysis, the right shoulder joint properties improved in the following aspects: elimination of shoulder joint pain (according to VAS 0), increase of the active range of shoulder joint movement into ventral flexion by 10°, abduction by 10° and external rotation by 15°. For passive range of motion, the same values were achieved as for flexion and abduction, and for external rotation, the range of motion was increased by 20°. There was also an improvement in muscle strength, which was assessed using the Janda muscle testing criteria. The main improvements were in scapular abduction, shoulder joint abduction, external and internal rotation (all by 1 degree). The patient himself rated the results of the therapy positively. Conclusion: The results point to the importance of addressing the issue of rotator cuff tears as a complex entity and not purely as a glenohumeral joint issue. Conservative physiotherapy appears to be an effective method in patients with recurrent pain and limited range of motion after rotator cuff rupture.
