Laboratory Diagnosis of Celiac Disease

Abstract:

Celiakie je autoimunitní onemocnění vyvolané intolerancí glutenu v tenkém střevě. Jedná se o geneticky podmíněnou ztrátu orální tolerance na štěpné produkty (peptidy) glutenu, které vyvolávají u disponovaných jedinců trvalou tvorbu protilátek. V posledních letech výskyt celiakie celosvětově stoupá. Diagnostika celiakie je založená především na vyšetření sérových autoprotilátek proti tkáňové transglutamináze a protilátek proti endomyziu ve třídě IgA. Při selektivním IgA deficitu je třeba vyšetřit sérové autoprotilátky ve třídě IgG. Dalším důležitým vyšetřením je při podezření na celiakii biopsie sliznice tenkého střeva. Jedinou doposud využívanou léčbou celiakie je bezlepková dieta.

Abstract:

Celiac disease is an autoimmune disease caused by intolerance to gluten in the small intestine. It is a genetically determined loss of oral tolerance in the fission products (peptides) of wheat gluten, which cause in predisposed individuals sustained antibody production. In recent years, the incidence of celiac disease has increased worldwide. Diagnosis of celiac disease is based primarily on the detection of serum autoantibodies against tissue transglutaminase and endomysium antibodies in the IgA class. In selective IgA deficiency serum IgG antibodies should be investigated. Another important investigation, when testing suspected celiac disease, is small intestine biopsy. The only previously used treatment of celiac disease is gluten-free diet.
