Determination of fatty acids in phospholipid sperm membrane and oxidative stress status in patients of reproductive centre

Abstract:

V dnešní době se stává mužská neplodnost velmi častým problémem i u mladých mužů. Mezi důležité patologické mechanismy vedoucí k mužské neplodnosti patří poškození membrány spermií tzv. lipoperoxidací. Fosfolipidová membrána lidských spermií je obzvlášť náchylná k oxidačnímu poškození kvůli vysokému obsahu polynenasycených mastných kyselin. Vznik lipoperoxidů a metabolitů lipoperoxidace je kaskádovitý děj iniciovaný volnými radikály. Tyto toxické produkty negativně ovlivňují průběh spermatogeneze, čímž dochází ke snížení počtu spermií, poškození jejich membrány a snížení motility a ztrátě schopnosti spermie podstoupit akrozomovou reakci a oplodnění. Jako ukazatel lipoperoxidace je nejčastěji stanovován malondialdehyd. Jedná se o genotoxickou a velmi reaktivní látku. Tabákový kouř je zdrojem volných radikálů, které jsou příčinou snížené pohyblivosti spermií. Kyselina askorbová a močová jsou významnými antioxidanty seminální plazmy, které chrání spermie před působením volných radikálů. Hladiny malondialdehydu, kyseliny askorbové a močové byly stanoveny metodou vysokoúčinné kapalinové chromatografie. Hladiny mastných kyselin fosfolipidů membrány spermií byly stanoveny plynovou chromatografií. Do studie bylo zahrnuto 113 pacientů Centra asistované reprodukce. Pacienti byli rozděleni na základě vyplněných dotazníků na kuřáky a nekuřáky, a na základě příručky Světové zdravotnické organizace (1999) na normozoospermiky, astenoteratozoospermiky, oligoastenoteratozoospermiky, oligoteratozoospermiky a teratozoospermiky. Statistickou analýzou byly vyšetřeny rozdíly v hladinách sledovaných parametrů u skupin pacientů a korelace mezi naměřenými hladinami a parametry spermiogramu. Rozdíly mezi koncentracemi stanovovaných látek u kuřáků a nekuřáků byly statisticky nevýznamné. Statisticky významný rozdíl byl nalezen mezi skupinou normozoospermiků a oligoastenoteratozoospermiků v případě procentuálního zastoupení kyseliny arachidonové. Mezi naměřenými hladinami sledovaných analytů a parametry spermiogramu byly nalezeny statisticky významné korelace.

Abstract:

Impairment of spermatogenesis and defective sperm function are the most prevalent cause of male infertility. The spermatozoa membrane structure plays a key role in successful fertilization. The sperm membrane phospholipids, mainly polyunsaturated fatty acid, are important for its fluidity, flexibility and functional activity. On the other hand, they are the most attacked substrate in lipid peroxidation processes. The initial products of polyunsaturated fatty acid oxidation are free radicals. These toxic products damage the process of spermatogenesis; thereby reducing the number of sperm cells and sperm motility, causing an inability to undergo the acrosome reaction and fertilize. Malondialdehyde is one of the final products of lipid peroxidation in seminal plasma. It is a highly reactive and genotoxic agent. Smoking, a potential free radicals source, is thought to be one of the risk factors raising their levels and may be responsible for sperm immobility and inability to fertilize. Essential role plays an antioxidant mechanism in the seminal plasma. It is represented by ascorbic and uric acid in this study. These molecules are scavengers of reactive oxygen species and protect spermatozoa against free radicals and damage. Malondialdehyde, ascorbic acid, and uric acid levels were measured using high performance liquid chromatography. Sperm phospholipid fatty acids were determined by gas chromatography. One hundred and thirteen male partners of couples requesting a fertility evaluation were selected to participate in the research. The patients were divided into groups according to the completed questionnaire, into smokers and non-smokers, and according to World Health Organization guidelines (1999) into normozoospermic, asthenoteratozoospermic, oligoasthenoteratozoospermic, oligoteratozoospermic, and teratozoospermic groups. The differences between groups and correlations between determined analytes and spermiogram parameters were established. There were not found statistically significant differences between smokers and non-smokers in levels of measured analytes. A difference in percentage of arachidonic acid between normozoospermic and oligoasthenoteratozoospermic men was significant. There were found statistically significant correlations between levels of measured analytes and spermiogram parameters.
