Nursing diagnosis reduced volume of body fluids in the enderly.

Abstract:

Téma této práce je ošetřovatelská diagnostika sníženého objemu tělesných tekutin u seniorů. Hlavním cílem práce je popsat postup ošetřovatelské diagnostiky k dehydrataci seniorů v NNP. Mezi další cíle práce patří zjistit, stav hydratace a bilanci tekutin u seniorů v NNP, zjistit, způsoby diagnostiky sníženého objemu tělesných tekutin u seniorů a zjistit, intervence sester ke sníženému objemu tělesných tekutin u seniorů. Teoretická část práce se zabývá obecnou charakteristikou ošetřovatelského procesu, krátkou definicí fáze posuzování a ošetřovatelskou diagnostikou. V této části je popsána ošetřovatelská diagnóza snížený objem tělesných tekutin, její specifika k seniorům a jsou zde podrobně popsány určující znaky a související faktory. V empirické části byl použit průzkum, jehož cílem bylo zjistit stav hydratace a bilance tekutin u seniorů. V průzkumu jsme se dále zabývali postupem jak správně určit ošetřovatelskou diagnózu. Ke zpracování empirické části jsme použili metodu kazuistiky doplněnou o nepřímé pozorování, rozhovor s klientem, rozhovor s ošetřujícím personálem a analýzou dokumentace pacienta. Byli vybrání tři klienti starší 65- ti let hospitalizovaní v NNP Ryjice, u kterých došlo ke sníženému objemu tekutin v průběhu hospitalizace nebo u kterých byl snížený objem tekutin prvotní příčinou zhoršeného zdravotního stavu. Pomocí průzkumu jsme zjistili že, ve všech třech případech byl příčinou sníženého objemu tělesných tekutin aktivní pokles objemu tělesných tekutin, dále že, dostatečně hydratován byl jen jeden klient a další dva měli stav hydratace snížený. Bilance tekutin byla ve dvou případech snížena a v jednom případě v pořádku. Z průzkumu vyplynulo že, sestry pracují metodou ošetřovatelského procesu, nevyužívají klasifikaci ošetřovatelských diagnóz dle NANDA International, ale označení jednotlivých diagnóz tomuto systému odpovídá, sestry využívají ke své práci velké spektrum škál a ošetřovatelských testů.

Abstract:

The theme of this work is the nursing diagnosis decreased body fluid volume in the elderly. The main goal of this paper is to describe the process of nursing diagnosis for dehydration in the elderly NNP. Other objectives include work to determine the state of hydration and fluid balance in the elderly in the NNP, identify methods of diagnosis reduced the volume of body fluids in elderly patients and to identify nursing interventions to reduced volume of body fluids in elderly patients. The theoretical part deals with general characteristics of the nursing process, a short definition phase of assessment and nursing diagnosis. This section describes the nursing diagnosis decreased the volume of body fluids, the elderly and the specifics are described in detail defining characteristics and related factors. In the empirical part of the survey was used, in order to establish the state of hydration and fluid balance in the elderly. In a survey I have also discussed the procedure to correctly identify the nursing diagnosis. The empirical part of the process I used the method of case histories supplemented by indirect observation, interview with the client, interview with the nursing staff analysis and documentation of the patient. , Three clients were over 65 - years in the NNP Ryjice hospitalized patients who had reduced the volume of fluid during hospitalization or for which the reduced volume of fluid the primary cause of deterioration in health. With the survey we found that, in all three cases, the cause of the reduced volume of body fluids active decrease of body fluids, as well as that, sufficiently hydrated was only one client and another two had a reduced state of hydration. Fluid balance was reduced in two cases and one case in order. The survey showed that, by working nurses nursing process, do not use the classification of nursing diagnoses according to NANDA International, but the description of each diagnosis corresponds to this system, nurses use their work to a large range of scales and nursing tests.
