Oral health status, tooth wear and periodontal lesions in patients with gastro-oesophageal reflux disease

Abstract:

Cílem této retrospektivní studie bylo přiblížit problematiku refluxní nemoci jícnu a hlavních projevů v dutině ústní. Zaměřili jsme se na poškození tvrdých zubních tkání a parodontu, které jsme pro objektivní zhodnocení porovnali s poškozením ve zdravé kontrolní skupině. Snažili jsme se zohlednit, co možná nejvíce predisponujících faktorů pro rozvoj poškození tvrdých a měkkých tkání dutiny ústní.Bylo vyšetřeno 90 pacientů, z toho 30 žen a 20 mužů s refluxní nemocí jícnu (GERD).Z výsledků deskriptivní statistiky vyplývá, že průměrné KPE bylo vyšší ve sledované skupině.Výsledky gingiválního indexu PBI se jeví jako vysoce statisticky významné na 0,1 % hladině významnosti.Pro celkové CPITN se výsledky jeví jako statisticky významné na 0,1 % hladině významnosti.Rozdíl mezi poškozením frontálních zubů byl zjištěn na 0,1 % hladině významnosti. Naměřené hodnoty ukazují hraniční produkci slin a snížené pH slin.Výsledky ukazují, že čtyři pacienti (16,7%) měli vysoké riziko zubního kazu, a 9 (37,5%) pacientů nízké vzhledem k počtu kmenů Streptococcus mutans. Tři (12,5%) pacienti měli vysoké riziko vzniku zubního kazu a 11 pacientů nízké (45,83%) vzhledem k počtu kolonií laktobacilů.

Abstract:

The aim of these case-control study was to present a complex oral health status including salivary factors, microbial analysis and periodontal and hygiene indices in patients with gastro-oesophageal reflux disease (GERD). The patient cohort consisted of 90 patients. 50 patients with gastroesophageal reflux disease GERD (30 women, 20 men) with mean age 45.8 years and 40 controls CG (16 women, 24 men) with mean age 44.3 were investigated.The DMFT index was higher in GORD group (mean 19.14, median 20) of which was D: 0.84, M: 6.02, F: 12.32 and control group (mean, 16.48, median 17) of which was D: 1.125, M: 3.75, F: 11.6 but without any statistical significance.Statistical evaluation of PBI index of obtained data revealed statistically significantly higher values in the study group (p <0.001).CPITN index was statistically higher in GERD group.The significant difference between group comparasion of tooth wear using Pearson Chi-square test for all teeth was showed (p<0.00001). This significance was verified also using Mann - Whitney U test.Basal flow rate has a boundary value and the stimulated flow rate in patients with active gastro-oesophageal reflux disease is lower than normal with low pH for both. For buffering capacity exhibited a low buffering capacity on stimulation.For the caries risk evaluation, Dentocult SM® and Dentocult LB® tests were used. The low caries risk was evaluated.
