Impact of bariatric surgery on hepatic statosis

Abstract:

Úvod: Nealkoholové ztučnění jater zahrnuje spektrum patologických stavů od prosté jaterní steatózy přes nealkoholovou steatohepatitidu, různý stupeň fibrózy, až po jaterní cirhózu se všemi jejími komplikacemi. Mezi strategie léčby patří dieta, hubnutí, úprava životního stylu a užívání léků ke snížení inzulínové rezistence a zvládnutí jaterní fibrózy. Bariatrická chirurgie je při léčbě NAFLD preferována před jinými postupy snižování hmotnosti. Metody: Cílem studie bylo prospektivně zhodnotit steatózu jater u 72 pacientů, kteří podstoupili bariatrickou operaci na Chirurgické klinice Fakultní nemocnice Ostrava v letech 2016 až 2018. Hodnoceny byly antropometrické údaje, laboratorní parametry a skórovací systémy steatózy jater (index ztukovatění jater FLI, NAFLD skóre fibrózy, Fibrosis 4 index, a poměr aspartátaminotransferáza/alanínaminotransferáza). Získaná data byla hodnocena před operačním výkonem a rok od operace. Srovnávány byly vzájemně 2 skupiny pacientů - skupina pacientů, u kterých došlo 1 rok od operace k redukci hmotnosti o 10,0 % (skupina I) a skupina pacientů, u kterých k redukci hmotnosti nedošlo (skupina II). Výsledky: Hodnocení prokázalo určitou formu steatózy jater u všech 72/72 (100,0 %) pacientů. Dle zhodnocení redukce hmotnosti o 10,0 % tvořilo skupinu I 64/72 (88,9 %) pacientů a skupinu II 8/72 (11,1 %) pacientů. Ve skupině I došlo k redukci průměrné váhy ze 124,0 kg na 90,0 kg (o 27,4 %). Ve skupině II došlo k redukci váhy ze 136,0 kg na 128,5 kg (o 5,9 %). Ve skupině I došlo k redukci BMI z předoperačního 43,24 na 31,0 kg/m2 (o 28,3 %). Ve skupině II se upravilo BMI z předoperačního 49,4 na 44,2 kg/m2 (o 10,5 %). Skupiny byly vzájemně porovnány i ve skórovacích systémech steatózy jater. Ve skupině I došlo ke snížení steatózy dle FLI skóre, ve skupině II nedošlo k úpravě. Dle NAFLD skóre fibrózy ve skupině I došlo k zlepšení fibrózy u 16/64 pacientů (25,0 %). Ve skupině II došlo ke zlepšení fibrózy u 2/8 (25,0 %) Při hodnocení dle FIB 4 skóre fibrózy ve skupině I nedošlo ke zlepšení fibrózy, ve skupině II nedošlo k žádným změnám. Podle poměru AST/ALT ve skupině I došlo pooperačně k úpravě u 2/64 (3,1 %) pacientů a ve skupině II u 1/8 (12,5 %) pacientů. K úpravě diabetu mellitu 2. typu došlo u 11/21 pacientů (52,3 %). Závěr: Z daných výsledků vyplývá, že NAFLD je součástí obézních pacientů. Bylo prokázano, že bariatrická chirurgie má efekt nejen na snížení váhy daných pacientů, ale rovněž má i efekt metabolický s úpravou komorbidit.

Abstract:

Introduction: Non-alcoholic fatty liver disease includes a spectrum of pathological conditions from simple hepatic steatosis to non-alcoholic steatohepatitis, varying degrees of fibrosis, to liver cirrhosis with all its complications. Treatment strategies include diet, weight loss, lifestyle modification, and medication to reduce insulin resistance and manage liver fibrosis. Bariatric surgery is preferred over other weight-loss procedures in the treatment of hepatic steatosis. Methods: The aim of the study was to prospectively evaluate hepatic steatosis in patients who underwent bariatric surgery at the Surgical Department of the Ostrava University Hospital. Anthropometric data, laboratory parameters and liver steatosis scoring systems, namely Fatty Liver Index (FLI), NAFLD fibrosis score, Fibrosis 4 index, and aspartat aminotransferase/alanin aminotransferase ratio were evaluated. The patients data were evaluated before the operation and one year after the operation in a total of 72 obese patients who underwent bariatric surgery between 2016 and 2018. 2 groups of patients were compared with each other - a group of patients who achieved weight loss of 10 % or more of their body weight 1 year after the operation (group I) and and who did not achieve weight loss (group II). Results: According to the assessment of weight reduction by 10,0 %, group I consisted of 64/72 (88.9 %) patients and group II 8/72 (11.1 %) patients. In group I, the average weight was reduced from 124.0 kg to 90.0 kg (by 27.4 %). In group II, there was a weight reduction from 136.0 kg to 128.5 kg, (by 5.9 %). In group I, there was a reduction in BMI from preoperative 43.24 to 31.0 kg/m2, (by 28.3 %). In group II, the BMI was adjusted from preoperative 49.4 to 44.2 kg/m2 (by 10.5 %). The groups were also compared with each other in the liver steatosis scoring systems. In group I, there was a reduction in steatosis according to the FLI score, in group II, there was no adjustment.According to the NAFLD fibrosis score in group I, fibrosis improved in 16/64 patients (25.0 %), in group II in 2/8 (25.0 %). When evaluating according to the FIB 4 fibrosis score in group I, there was no improvement in fibrosis and there were no changes in group II. According to the AST/ALT ratio, 2/64 (3.1 %) patients in group I and 1/8 (12.5 %) patients in group II improved postoperatively. Type 2 diabetes mellitus was corrected in 11/21 patients (52.3 %). Conclussion: The results show that obesity is associated with an increased risk of nonalcoholic fatty liver disease. Bariatric surgery has an effect on reducing the weight of patients and also has a metabolic impact on related comorbidities.
