Pain management in patients on artificial lung ventilation in post-acute intensive care

Abstract:

Tato diplomová práce se zabývá posuzováním bolesti u pacientů na UPV hospitalizovaných na oddělení následné intenzivní péče. Teoretická část je zaměřena na pacienta na pracovišti intenzivní péče a přibližuje i pracoviště následné intenzivní péče. Popisuje problematiku umělé plicní ventilace a také obtíže spojené s komunikací s těmito pacienty. Objasňuje bolest jako pojem, historii léčby bolesti, její druhy a možnosti současné léčby, a úlohu sestry v managementu bolesti. Cílem výzkumné části práce je pozorování a zaznamenávání bolesti pacientů spojené s bolestivými intervencemi v jednotlivých časových intervalech za pomocí hodnotící škály FLACC. Dále se v práci věnuji mapování zkušeností a názorů sester na problematiku managementu bolesti u ventilovaných pacientů na oddělení NIP. Byl zjištěn statisticky významný rozdíl mezi hodnotami naměřenými před intervencí a při intervenci, což ukazuje na nedostatečnou analgezii při bolestivých výkonech. Dle výsledků dotazníků většina sester přehodnocuje bolest po celou dobu směny, využívají nefarmakologické možnosti tlumení bolesti a s nastavenou výší analgezie u pacientů je spíše spokojena.

Abstract:

This diploma thesis deals with the assessment of pain in patients on artificial lung ventilation hospitalized in the department of follow-up intensive care. The theoretical part is focused on the patient in the workplace of intensive care and also approaches the work place of subsequent intensive care. It describes the issue of artificial lung ventilation and also the difficulties as sociated with communication with these patients. It explains pain as a concept, the history of pain treatment, its types and possibilities of current treatment, and the role of the nurse in pain management. The aim of the research part of the work is to observe and record the pain of patients as sociated with painful interventions in individual time intervals using the FLACC rating scale. Furthermore, to map the experience and opinions of nurses on the issue of pain management in ventilated patients in the NIP department. A statistically significant difference was found between the values measured before the intervention and at the intervention, which indicates insufficient analgesia during painful procedures. According to the results of the questionnaires, most nurses re-evaluate the pain throughout the shift, use non-pharmacological options for pain relief and are rather satisfied with the set amount of analgesia in patients.
