Global methylation profiles and comprehensive analysis of gene fusions as a basis for therapeutic planning in pediatric oncology

Abstract:

Koncept personalizované medicíny nabízí nové terapeutické možnosti u pediatrických onkologických pacientů, u kterých selhaly standardně využívané léčebné modality, případně u nově diagnostikovaných pacientů s nádory, které se vyznačují špatnou prognózou (tzv. vysoce rizikové solidní nádory). Základem tohoto přístupu je provedení detailní molekulárně-genetické charakterizace nádorové tkáně, která poskytuje informaci o možných terapeutických cílech. Pro co nejdůkladnější charakterizaci je nezbytné se zaměřit nejen na změny v kódujících oblastech genomu, ale také na studium změny genových expresních profilů a detekci strukturních přestaveb genů a epigenetických alterací. V rámci této diplomové práci byla provedena molekulární klasifikace pacientů s meduloblastomem na základě dat získaných analýzou transkriptomu a metylačních profilů DNA. Kromě klasifikace byla pro každého pacienta získána také informace o změnách genové exprese v nádorové tkáni. Na základě těchto informací byla navržena možná terapie v podobě cílené léčby. U pacientů s pediatrickými nízkostupňovými gliomy byla provedena analýza fúzních genů. U těchto pacientů bylo již dříve provedeno celoexomové sekvenování, které neodhalilo cílitelné alterace. U každého z nich se podařilo úspěšně detekovat a verifikovat klinicky relevantní fúzní gen a na základě literatury byly dále navrženy terapeutické možnosti.

Abstract:

Personalized medicine is a therapeutic concept that offers new therapeutic possibilities to pediatric oncological patients who have failed standard treatment modalities, or in newly diagnosed patients with tumors that are characterized by poor prognosis (so-called high-risk solid tumors). The key is detailed molecular genetic characterization of tumor tissue providing information about possible therapeutic targets. It is necessary to focus not only on changes in the coding regions of the genome but also on the alterations in gene expression profiles, on the detection of structural rearrangements of genes and the epigenetic alterations. In this diploma thesis, medulloblastoma classification was performed based on data obtained from transcriptome and DNA methylation profiling. In addition to the classification, the information about gene expression in tumor tissue was obtained. Based on this information, therapeutic possibilities were recommended. In this diploma thesis, there was also performed fusion genes analysis in tumor tissue of patients with diagnosed pediatric low-grade gliomas. Previously, whole-exome sequencing was performed in these patients, but no therapeutically actionable variants were detected. One fusion gene that was relevant to diagnose was successfully detected and verified in each patient. Then, based on the literature therapeutical options were proposed.
