Cytokines in atherosclerosis

Abstract:

Ateroskleróza je chronický zánětlivý proces postihující cévní stěnu. Proces rozvoje aterosklerózy je složitý a probíhá postupně od stadia endotelové dysfunkce, přes prostup monocytů do subendotelového prostoru, transformaci hladkých svalových buněk, vznik aterosklerotických lézí, až po jejich destabilizaci a stádium trvajícího zánětu. Souhra imunitních mechanismů vedoucích k rozvoji aterosklerózy je umožněna řadou cytokinů. V počátečních fázích aterosklerózy hrají důležitou úlohu především chemokiny (např. MCP-1), které napomáhají prostupu monocytů do cévní stěny. Diferenciace monocytů na makrofágy je závislá na faktoru M-CSF. K expresi adhezních molekul na povrchu endotelových buněk a scavenger receptorů na povrchu makrofágů přispívá TNF-alfa, který tak podporuje vznik pěnových buněk. Hladké svalové buňky podléhají změnám fenotypu především vlivem některých růstových faktorů (např. PDGF). Z interleukinů jsou velmi důležité prozánětlivé IL-1 a IL-6, jejichž vysoká koncentrace v pozdějších stádiích podporuje destabilizaci aterosklerotických plaků. Podstatnou roli v ateroskleróze má také prozánětlivý IFN-gama. Za antiaterogenní faktory jsou považovány např. TGF-beta a IL-10.

Abstract:

Atherosclerosis is a chronic inflammatory process which affects the vascular wall. The process of developing atherosclerosis is complex and progresses gradually from the stage of endothelial dysfunction, to monocyte penetration into the subendothelial area, from the transformation of smooth muscle cells, to the occurrence of atherosclerotic lesions and after their destabilization and the stage of unresolved inflammation. The interaction of immune mechanisms leads to the development of atherosclerosis and is facilitated by a number of cytokines. In the early stages of atherosclerosis primarily chemokines (for instance MCP-1) play a key role, they help the monocyte recruitment into the vascular wall. The differentiation of monocytes to macrophages depends on M-CSF. The expression of adhesion molecules on the surface of endothelial cells and scavenger receptors on the surface of macrophages, contribute to TNF-alfa, which supports the formation of foam cells. Smooth muscle cells are subjected to phenotypic changes, mainly due to some growth factors (for instance PDGF). Pro-inflammatory IL-1 and IL-6 are very important interleukins; their high concentration in later stages support the destabilization of atherosclerotic plaques. Pro-inflammatory IFN-gama has also an essential role in atherosclerosis. For anti-atherogenic factors are considered for instance TGF-beta and IL-10.
