Determination of fibronectin in different diagnostic groups

Abstract:

Úvod a cíle práce: Fibronektin je glykoprotein tělních tekutin, vyskytující se na povrchu buněčných membrán a je součástí extracelulární matrix. Účastní se buněčné adheze, proliferace, migrace a diferenciace. Cílem práce bylo stanovit jeho hladinu v séru/plazmě u různých diagnostických skupin a posoudit jeho korelaci mezi jednotlivými diagnostickými skupinami a biomarkery charakterizujícími příslušné onemocnění. Materiál a metodika: Do studie bylo zařazeno 60 pacientů z Fakultní nemocnice Ostrava s podezřením na sepsi, preeklampsii a nádorové onemocnění, přičemž každá diagnostická skupina byla zastoupena 20 probandy. Kontrolní skupinu tvořilo 20 probandů Krevního centra reprezentující zdravou populaci. Fibronektin byl stanoven nefelometricky analyzátorem Atellica NEPH 630 (Siemens). Výsledky: Na základě statistického zpracování dat byla prokázána přímá závislost mezi koncentrací fibronektinu a poměrem PE-R. Nebyla prokázána korelace mezi hladinou FN a markerem sepse, PCT. Současně nebyla zjištěna korelace s koncentrací fibronektinu a nádorovými biomarkery, AFP, CEA, CA 19-9 a CA 125. Pomocí Kruskal Wallisova testu byl zjištěn rozdíl v koncentracích fibronektinu u kontrolní skupiny a skupiny pacientů s nádorovým onemocněním a sepsí, současně u skupiny pacientů s preeklampsií a nádorovým onemocněním a sepsí. Závěr: Fibronektin je nový potenciální biomarker se širokým uplatněním nejen v klinické biochemii. Naše výsledky bohužel nejsou jednoznačně v souladu s publikovanými studiemi, důvodem může být malý soubor vzorků, jiné patologické stavy ovlivňující hladinu FN či malnutrice. Pro dosažení validnějších údajů by bylo vhodné soubor rozšířit a v případě studia nádorových onemocnění se zaměřit na určitý typ nádoru.

Abstract:

Introduction and the main aims of this bachelor thesis: Fibronectin is a glycoprotein of body fluids, occurs on the surface of cell membranes and is part of the extracellular matrix. They are involved in cell adhesion, proliferation, migration, and differentiation. The aim of the study was to determine its serum/plasma level in various diagnostic groups and to assess its correlation between individual diagnoses and biomarkers characterizing the relevant disease. Material and methods: The study includes 60 patients from the University Hospital Ostrava with suspected sepsis, preeclampsia, and cancer, with each diagnostic group represented by 20 probands. The control group consists of 20 Blood Centre probands representing a healthy population. Fibronectin was determined by using nephelometry on the Atellica NEPH 630 analytical system (Siemens). Results: Based on statistical data processing, a direct relationship between fibronectin concentration and PE-R ratio was demonstrated. No correlation was found between the level of FN and the marker of sepsis, PCT. At the same time, no correlation was found with fibronectin concentration and tumour biomarkers, AFP, CEA, CA 19-9, and CA 125. Using the Kruskal-Wallis test, a difference in fibronectin concentrations was found in the control group and the group of patients with cancer and sepsis. preeclampsia and cancer and sepsis. Conclusion: Fibronectin is a new potential biomarker with a wide application not only in clinical biochemistry. Unfortunately, our results are not clearly in accordance with published studies, the reason may be a small set of samples, other pathological conditions affecting the level of FN or malnutrition. To achieve more valid data, it would be appropriate to expand the set and, in the case of cancer studies, to focus on a certain type of tumor.
