The importance of molecular genetic testing of BRCA1, BRCA2 and other genes in breast and ovarian cancer patients

Abstract:

Rakovina prsu a vaječníků patří mezi nejčastější onkologická onemocnění u žen v ČR. Majoritní část představují sporadické nádory. Dědičná forma je méně častá, ale klinicky velmi významná, přičemž bývá spojena s mutacemi v tumor supresorových genech BRCA1 a BRCA2. V současnosti s rozvojem nových molekulárních metod, zejména sekvenování nové generace, se ale ukazuje, že podíl hereditárních karcinomů byl značně podceňován. Například u karcinomu ovaria představuje podíl nosiček mutací v genech BRCA1 a BRCA2 víc jak 10%, studie z poslední doby navíc odhalily patogenní varianty v dalších nádorových predispozičních genech až u 40% těchto pacientek. Podobný, i když možná ne tak výrazný trend, se předpokládá i u nádorů prsu. Cílem bakalářské práce bude představit dostupné metody analýzy zejména genů BRCA1/2, ale i ostatních relevantních genů. Teoretická část se bude zabývat záchytem nejčastějších mutací v genech BRCA1/2 na somatické a germinální úrovni a popsáním jejich významu pro klinickou praxi. Následně podá přehled dalších genů, ve kterých byly nalezeny kauzální varianty u pacientů s karcinomem prsu a ovaria a z dostupných studií stanoví četnost záchytu v jednotlivých genech. V rámci praktické části, budou popsány metodiky, které se využívají pro standardní klinickou analýzu BRCA1/2 genů na našem pracovišti a záchyt patogenních variant v těchto genech.

Abstract:

Breast and ovarian cancer is one of the most common oncology diseases in women in the Czech Republic. The majority are sporadic tumors. The inherited form is less common but clinically significant, and is associated with mutations in tumor suppressor BRCA1 and BRCA2 geenes. At present, with the development of new molecular methods, especially new generation sequencing. It appears that the proportion of hereditary carcinomas has been greatly underestimated. For ovarian karcinoma, for example, the proportion of BRCA1 and BRCA2 mutation carriers is more than 10%, and recent studies have revealed pathogenic variants in others tumor predisposition genes in up to 40% of these patients. A similar, though perhaps not so pronounced trend is also expected breast cancer. The aim of the bachelor thesis is to introduce molecular analysis especially of BRCA1/2 genes, as well as other relevant genes. The theoretical part will deal with the detection of the most frequent mutations in BRCA1 / 2 genes mainly at the germinal level and, the describing of their signifikance for clinical practice. Within the framework of the practical part, we will describe the methodologies used for standard clinical analysis of BRCA1 / 2 genes in our workplace, and the detection of pathogenic variants in these genes.
