Sledování exprese znaku CD38 na CD8+ T-lymfocytech v diagnostice cytomegalovirové infekce u pacientů po orgánové transplantaci

Monitoring of expression of CD38 on CD8+ lymphocytes in diagnosis of cytomegalovirus infection in patients after organ transplantation

Abstract:

Pacienti po transplantaci solidních orgánu jsou v prvních měsících po transplantaci nejvíce ohroženi infekčními a rejekčními komplikacemi. Jedním z běžných oportunních patogenů, které nemocné mohou ohrozit, je cytomegalovirus. Cílem této práce bylo stanovení klinického významu sledování procenta CD38high z CD3+CD8+ T-lymfocytů pomocí průtokové cytometrie jako nástroje pro detekci aktivace imunitního systému v průběhu CMV infekce. Analýza byla provedena na souboru pacientů po transplantaci ledvin (n = 269) a transplantaci jater (n = 162) v době jejich hospitalizace. Z těchto nemocných byla diagnóza cytomegalovirové infekce stanovena u 12 osob po transplantaci ledviny (4,5%) a u 10 (6,2%) po transplantaci jater. Při využití arbitrární hranice 20% CD38high z CD3+CD8+ T-lymfocytů a CMV diagnózy jako referenčního parametru byla stanovena senzitivita metody 100% pro oba soubory pacientů. Pro soubor pacientů po transplantaci ledviny byla specificita testu stanovena 91% procent, u pacientů po transplantaci jater 48%. Metoda stanovení procenta CD38high z CD3+CD8+ vykazovala vysokou negativní prediktivní hodnotu – 100% v obou souborech pacientů; pozitivní prediktivní hodnoty byly nízké – 34% u pacientů po transplantaci ledvin a 11% po transplantaci jater. U 4/12 pacientů po transplantaci ledvin a 6/10 pacientů po transplantaci jater nebyl detekován počet kopií CMV DNA ≥1000 kopií/ml periferní krve. Diagnóza CMV infekce byla založena na jiných klinických či laboratorních příznacích a procento CD38high z CD3+CD8+ lymfocytů přesahující arbitrární limit podpořilo rozhodnutí pro zahájení antivirové terapie. U obou souborů pacientů s CMV infekcí byly nalezeny statisticky významná korelace mezi sledovaným procentem CD38high z CD3+CD8+ lymfocytů a procentem HLA-DR+ z CD3+, procentem CD8+ z CD3+ lymfocytů, procentem CD4+ z CD3+ lymfocytů, poměrem CD3+CD4+/CD3+CD8+ lymfocytů a absolutním počtem leukocytů v krvi. Tyto parametry jsou typickými imunologickými ukazateli virové infekce a jejich korelace podporují smysl využití sledování procenta CD38high z CD3+CD8+ T-lymfocytů pro diagnostiku CMV infekce. Monitorování procenta CD38high z CD3+CD8+ lymfocytů pomocí průtokové cytometrie je rychlá a jednoduchá metoda s vysokou reprodukovatelností a jeví se, vzhledem ke svému komplementárnímu charakteru, vhodnou jako doplněk již konvenčních a rutinně využívaných imunologických a virologických metod, obzvláště u pacientů v riziku orgánově specifických CMV komplikací.

Abstract:

In the first months the patients after solid organ transplantations are endangered mainly by infectious complications and graft rejection. Cytomegalovirus is one of the most significant common opportunistic pathogens, which can cause life threatening complications to the transplant patients. The goal of this study was to evaluate clinical significance of monitoring CD38high of CD3+CD8+ T-lymphocytes estimated by flow cytometry as a tool for detection of immune system activation during a CMV infection. Analysis was performed on group of patients after renal (n=269) and liver (n=162) transplantation during their hospitalization. The test was considered positive if the percentage of CD38high of CD8+ cell exceeded 20%. During post-transplantation hospitalization, the diagnosis of CMV infection was established in 12 (4,5%) patients with transplanted kidney and 10 (6,2%) with transplanted liver. Using clinical decision of treatment of the CMV infection as a reference value, the sensitivity of monitoring percentage of CD38high of CD3+CD8+ T-lymphocytes was 100% in both groups of patients. The test specificity was determined at 91% for the group of patients after renal transplantation and 48% for the patients after liver transplantation. Monitoring percentage of CD38high of CD3+CD8+ showed high negative predictive value – 100% in both groups of patients; but positive predictive value was low – 34% in patients after kidney transplantation and 11% in patients after liver transplantation. In 4/12 patients after kidney transplantation and 6/10 patients after liver transplantation, the amount of copies of CMV DNA ≥1000 copies/ml of peripheral blood was not detected. The diagnosis of CMV infection was based on clinical and laboratory signs and the percentage of CD38high of CD3+CD8+ T-lymphocytes above arbitrary limit supported the decision for antiviral treatment initiation. In both groups of patients with CMV infection statistically significant correlation between percentage of CD38high of CD3+CD8+ lymphocytes and a percentage of HLA-DR+ of CD3+, percentage of CD8+ of CD3+ lymphocytes, percentage of CD4+ of CD3+ lymphocytes, CD3+CD4+/CD3+CD8+ lymphocyte ratio and absolute leucocyte count in peripheral blood was found. These parameters are typical immunologic markers of viral infection and their correlations support significance of monitoring the CD38high of CD3+CD8+ T-lymphocytes for CMV infection diagnosis in post-transplantation period. Monitoring percentage of CD38high of CD3+CD8+ lymphocytes using the flow cytometry is a fast and simple method with high reproducibility and it seems that because of its complementary character it can be useful as an additional method to the conventional and routinely used immunologic and virologic tests mainly in patients in risk of tissue-invasive CMV disease.
