Mechanisms of impaired blood gas exchange in patients with acute respiratory failure and their influence on the effect of therapeutic interventions and clinical outcome

Abstract:

Hlavním tématem této disertační práce je snaha o zjištění, zdali lze neinvazivně, na podkla-dě kalkulace alveolo-arteriální diference kyslíku (AaDO2) odhadnout množství extravasku-lární plicní vody (EVLW) u pacientů s akutním respiračním selháním (ARDS) způsobeným koronavirovou infekcí SARS-CoV-2 (Coronavirus disease-2019, COVID-19). AaDO2 je globální parametr posuzující ventilaci a oxygenaci se základem v teoretických modelech výpočtu „ideálního alveolárního vzduchu“ ze 40. let 20. století. Původní klinický význam AaDO2 je v rozlišení extrapulmonální nebo intrapulmonální příčiny hypoxémie. V součas-ném pojetí se význam AaDO2 posunul k hodnocení míry poruchy výměny plynů při intra-pulmonálních patologiích v intenzivní péči. EVLW je kvantitativní ukazatel míry plicního edému. EVLW vyjadřuje množství vody (tekutiny), která se nachází mimo plicní oběh, tedy v intersticiu a alveolech. EVLW je dnes důležitou součástí hemodynamického moni-toringu v intenzivní péči u stavů spojených se zvýšenou permeabilitou alveolokapilární membrány (např. ARDS) nebo při poruše Starlingových sil na kapiláře (hydrostatický edém). Míra EVLW je nezávislým prediktorem klinického výsledku pacientů s ARDS. K měření EVLW se v dnešní době používá především invazivní metoda transkardiopulmo-nální termodiluce (TCPTD). TCPTD byla opakovaně validována a je považována za přes-nou. Limitací vyšetření je invazivita ale také dostupnost přístrojové techniky a cena. Pan-demie COVID-19 představovala riziko vyčerpání materiálních zdrojů a dostupné techniky invazivní hemodynamické monitorace. Za předpokladu, že by bylo možné EVLW odhad-nout měřením AaDO2, toto riziko by bylo nižší. Cílem této disertační práce je popsat vý-znam AaDO2, resp. EVLW ve vztahu k ARDS. Součástí disertační práce jsou komentáře k námi publikovaným odborným článkům k danému tématu.

Abstract:

The focus of this dissertation is to investigate whether the amount of extravascular lung water (EVLW) can be estimated non-invasively based on alveolar-arterial oxygen differ-ence (AaDO2) calculation in patients with acute respiratory distress syndrome (ARDS) caused by SARS-CoV-2 coronavirus infection (Coronavirus disease-2019, COVID-19). AaDO2 is a global parameter used to assess ventilation and oxygenation based on theoreti-cal models calculating “ideal alveolar air” from the 1940s. The original clinical significance of AaDO2 is in distinguishing extrapulmonary or intrapulmonary causes of hypoxemia. In the current concept, the meaning of AaDO2 has shifted to the assessment of the degree of gas exchange impairment in intrapulmonary pathologies in intensive care. EVLW is a quan-titative indicator of the degree of pulmonary edema. EVLW expresses the amount of water (fluid) that is outside the pulmonary circulation, i.e. in the interstitium and alveoli. EVLW is now an important part of hemodynamic monitoring in intensive care in conditions asso-ciated with increased alveolocapillary membrane permeability (e.g. ARDS) or in the failure of Starling forces on the capillary (hydrostatic edema). The level of EVLW is an independ-ent predictor of clinical outcome in patients with ARDS. Nowadays, the invasive method of transcardiopulmonary thermodilution (TCPTD) is mainly used to measure EVLW. TCPTD has been repeatedly validated and is considered accurate. The limitations of the method include not only its invasiveness but also the availability of equipment and cost. The COVID-19 pandemic posed a risk of depletion of material resources and available techniques for invasive hemodynamic monitoring. Assuming that EVLW could be estimat-ed by AaDO2 measurement, this risk would be lower. The aim of this dissertation is to de-scribe the significance of AaDO2 or EVLW in relation to ARDS. The dissertation includes comments on our published scientific articles on the topic.
