Laboratory diagnosis of rheumatoid arthritis

Abstract:

Tato bakalářská práce se zabývá problematikou stanovení protilátek proti cyklickému citrulinovému peptidu u pacientů s podezřením na postižení revmatoidní artritidou. Na začátku se práce věnuje obecné charakteristice imunitního systému, jeho mechanismům působení a jeho strukturním částem, jelikož je revmatoidní artritida onemocnění vzniklé na podkladě imunitní reakce zapříčiňující ztrátu tolerance vůči vlastním tkáním a způsobující zánět v kloubním prostoru. Při zánětlivé činnosti se do séra z kloubního prostoru uvolňuje citrulin, proti němuž se následně tvoří protilátky, které se staly důležitým nástrojem pro včasné stanovení diagnózy revmatoidní artritidy. Do výzkumu bylo zahrnuto 51 pacientů s podezřením na postižení revmatoidní artritidou, u většiny z nichž byla taktéž stanovena přítomnost revmatoidních faktorů. Výsledky stanovení jak revmatoidních faktorů, tak protilátek proti cyklickému citrulinovému peptidu, byly vzájemně klinicky porovnány za účelem potvrzení studií deklarujících výrazně nižší specifičnost revmatoidních faktorů pro revmatoidní artritidu než protilátky proti cyklickému citrulinovému peptidu, což bylo také potvrzeno. Práce se zaměřuje na popis a porovnání imunoanalytických metod s chemiluminiscenční detekcí, kterými lze protilátky proti cyklickému citrulinovému peptidu stanovit, jelikož číselné hodnoty výsledků stanovení na obou analyzátorech se velmi výrazně liší.

Abstract:

The purpose of is this bachelor thesis is to look at the determination issue of anti-cyclic citrulinated antibodies in patients who are possibly diagnosed with rheumatoid arthritis. The first part of the thesis is focused on the characteristics of the immune system, its mechanism and structure, as rheumatoid arthritis is caused by pathological immune reactions. The consequences of these reactions are lost of autotoleration and inflammation of joints. The inflammation causes creation of anti-cyclic citrulinated peptide antibodies, which is secreted of joint to blood. These antibodies are used to diagnose the rheumatoid arthritis early. The research included 51 patients suspected of rheumatoid arthritis. In most cases, there were determinated rheumatoid factors. The results of determination of rheumatoid factors and anti-cyclic citrulinated antibodies were consistent with studies proving the lower specificity of rheumatoid factors than anti-CCP for rheumatoid arthritis. Since the results of the two immunoanalytical techniques are varying, the bachelor thesis also embrace the description issues and comparison of two immunoanalytical techniques, which are used to determinate anti-cyclic citrulinated peptid antibodies.
