Importance of Hematological Parameter IPF (Immature Platelet Fraction) in the Laboratory Diagnosis

Abstract:

Retikulotrombocyty (mladé destičky) jsou markerem trombopoézy, za jejich pomoci lze monitorovat kinetiku trombocytů. Dříve byly retikulotrombocyty analyzovány pomocí flow cytometru, toto stanovení bylo však drahé, časově náročné a nestandardizované. Nyní je možné měřit retikulotrombocyty vyjádřené jako parametr IPF (immature platelet fraction) pomocí hematologického analyzátoru. Parametr IPF lze využít především v diferenciální diagnostice trombocytopenií a k monitorování trombopoézy po chemoterapiích a transplantacích kostní dřeně. Opakovatelnost metody měření IPF byla vyhodnocena jako relativně dobrá. Bylo stanoveno fyziologické rozmezí 1,4-4,3% a střední hodnota 2,9% parametru IPF. Bylo zjištěno, že parametr IPF je při pokojové teplotě v průběhu 12 hodin stabilní. Bylo potvrzeno, že u pacientů po chemoterapii je obnovení trombopoézy předznamenáno vzestupem parametru IPF několik dní před zvýšením produkce trombocytů. Cíle práce: 1. Rešerše literatury zaměřené na studium parametru IPF 2. Hodnocení parametru IPF v rutinní praxi. Za tímto účelem byla testována opakovatelnost metody měření, stabilita a fyziologické rozmezí parametru IPF 3. Monitorování parametru IPF u pacientů po chemoterapii a sledování kinetiky trombocytů

Abstract:

Reticulated platelets (young platelets) are a marker of the thrombopoiesis and they are useful for the monitoring of platelets kinetics. Young platelets have been measured by flow cytometry, however this determination has been expensive, time consuming and it has been never standardise. Now it is possible to measure reticulated platelets expressed as the parameter IPF (immature platelet fraction) by hematological analyzer. Parameter IPF can be used to a differencial diagnostic of thrombocytopenias and to the monitoring of thrombopoiesis following chemotherapy and bone marrow transplantation. The reproducibility of the IPF assay was relatively good. An IPF reference range was established as 1,4-4,3% with a mean of 2,9%. It was detected, that in room temperature is parameter IPF stable over 12 hours. It was validated, that in chemotherapy patiens is thrombopoiesis recovery preceded by a rise in IPF several days prior to increasing platelets production. The aims of this study: 1. Bibliographic search of literature focusing on research of parametr IPF 2. Evaluation of IPF in the routine laboratory practice. For this purpose were tested the reproducibility of the method, stability and reference range of parameter IPF 3. Monitoring of parameter IPF at patients following chemotherapy and monitoring of platelets kinetics.
