Identification and saturation of the needs of terminally ill patients

Abstract:

Zvýšená pozornost k biologickým, psychickým, sociálním a duchovním potřebám člověka je zvláště důležitá u terminálně nemocných pacientů. Hodnocení těchto potřeb je jednou z priorit moderní paliativní péče, jelikož jejich nedostatečná saturace výrazně snižuje kvalitu života umírajících. Je také vhodným indikátorem kvality poskytované péče. Tato diplomová práce je teoreticko - průzkumnou prací. Hlavním cílem teoretické části je sumarizace dohledaných poznatků v oblasti paliativní péče a s ní souvisejících aspektů; hodnocení a naplňování potřeb a koncept kvality života u terminálně nemocných pacientů. Zmíněná témata jsou doložená odbornou literaturou a relevantními výzkumy tuzemskými i zahraničními. V průzkumné části byl použit dotazník pro hodnocení potřeb pacientů v paliativní péči - PNAP (Patient Needs Assessment in Palliative Care), jehož autorkou je doc. PhDr. Radka Bužgová, Ph.D. z Lékařské fakulty Ostravské univerzity v Ostravě. Od autorky byl získán originál dotazníku a souhlas s jeho použitím. Prostřednictvím dotazníku byla zjišťována důležitost potřeb a jejich naplněnost/nenaplněnost u terminálně nemocných pacientů. Dotazník byl doplněn o demografické proměnné z důvodu zjištění jejich vlivu na hodnocení míry důležitosti a saturovanosti potřeb umírajících. Za nejdůležitější považovali pacienti potřeby v doméně respekt a podpora zdravotnického personálu (ZP), sociálních potřeb, autonomie, fyzických symptomů a smyslu života a smíření. Nejméně byli respondenti spokojeni se saturací potřeb v doméně fyzické a psychické. Naopak nejvíce byli spokojeni se saturací potřeb v doméně religiózních potřeb, respektu a podpory (ZP) a v doméně sociální. Vybrané demografické proměnné - věk, pohlaví, dosažené vzdělání a typ oddělení nemají v tomto souboru vliv na hodnocení jednotlivých položek dotazníku PNAP.

Abstract:

It is important to pay more attention to the physical, psychological, social and spiritual needs of terminally ill patients in the final phase of life. Assessing these needs is one of the priorities of modern palliative care, as their lack of saturation greatly reduces the quality of life of the dying. Assessing the named and fulfilled needs of patients is also an appropriate indicator of the quality of care provided. This diploma thesis is a theoretical - exploratory work. The main aim of the theoretical part is to summarize the knowledge gained in the area of palliative care and the related aspects; evaluation and fulfilment of needs and the concept of quality of life in terminally ill patients in the final stage of life. These topics are documented by professional literature and relevant domestic and foreign research. In the exploratory section, a PNAP (Patient Needs Assessment in Palliative Care) questionnaire was used to asses the needs of patients in palliative care, the author of which is doc. PhDr. Radka Bužgová, Ph.D. (LF OU). An original of the questionnaire was obtained from the author, as well as consent to its use. Through the questionnaire, the needs and their fulfilment/lack of fulfilment were assessed for terminally ill patients in the final stage of life. The questionnaire was supplemented by demographic variables in order to determine their impact on the importance and saturation of the needs of the dying. The patients considered the needs in domain of respect and support of medical staff, social needs, autonomy, physical symptoms, meaning of life and reconciliation the most important. Respondents were least satisfied with the saturation of needs in the domain of the possibility of sharing emotions and in the domain of physical symptoms. Adversely, they were most satisfied with the saturation of needs in the area of religious needs, respect and support of medical staff and in the social sphere domain. Selected demographic variables - age, gender, achieved education and type of department in this group do not affect the evaluation of the individual items of PNAP questionnaire.
