Abstract:

Tato bakalářská práce poskytuje ucelený přehled o Cushingově syndromu u koní, odborně označovaném jako dysfunkce pars intermedia hypofýzy (PPID). Jedná se o časté a progresivní neurodegenerativní onemocnění starších koní, jehož podstatou je ztráta dopaminergní inhibice v hypofýze, což vede k nežádoucí nadprodukci hormonů, zejména adrenokortikotropního hormonu (ACTH) a alfa-melanocyty stimulujícího hormonu (α-MSH). Mezi nejtypičtější klinické projevy patří hirsutismus, laminitida, úbytek svalové hmoty, nadměrné močení a pití či snížená imunita. Diagnostika onemocnění je komplikovaná zejména nenápadným nástupem příznaků a přirozeným sezónním kolísáním hormonů, proto se nejčastěji opírá o měření bazální koncentrace ACTH s využitím sezónně upravených referenčních limitů nebo o TRH stimulační test, jenž je užitečný pro detekci časných stádií. Lék pro celoživotní terapii je pergolid-mesylát, syntetický agonista dopaminových receptorů, který pomáhá obnovit inhibici a zmírňovat klinické příznaky. Farmakologická léčba však musí být bezpodmínečně doplněna o komplexní chovatelský management zahrnující úpravu výživy (zejména při přidružené inzulinové dysregulaci), pravidelnou péči o kopyta a chrup i stříhání srsti k prevenci tepelného stresu. Včasné rozpoznání nemoci a důsledná péče jsou tak zcela klíčové pro udržení dobré kvality života postižených zvířat.

Abstract:

This bachelor thesis provides a comprehensive overview of Equine Cushing's Syndrome, professionally referred to as Pituitary Pars Intermedia Dysfunction (PPID). It is a common and progressive neurodegenerative disease of older horses, the essence of which is the loss of dopaminergic inhibition in the pituitary gland, leading to an undesirable overproduction of hormones, particularly adrenocorticotropic hormone (ACTH) and alpha-melanocyte stimulating hormone (α-MSH). The most typical clinical signs include hirsutism, laminitis, muscle atrophy, excessive urination and drinking, and reduced immunity. Diagnosis of the disease is complicated mainly by the subtle onset of symptoms and natural seasonal fluctuations of hormones; therefore, it most often relies on measuring the basal concentration of ACTH using seasonally adjusted reference intervals, or the TRH stimulation test, which is useful for detecting early stages. The drug of choice for lifelong therapy is pergolide mesylate, a synthetic dopamine receptor agonist that helps restore inhibition and alleviate clinical signs. However, pharmacological treatment must unconditionally be supplemented by comprehensive husbandry management, including dietary adjustments (especially with associated insulin dysregulation), regular hoof and dental care, and clipping of the coat to prevent heat stress. Early recognition of the disease and consistent care are thus absolutely crucial for maintaining a good quality of life for the affected animals.
