The incidence of Candida dubliniensis in the clinical material and the importance of its identification

Abstract:

Diplomová práce je zaměřena na sledování výskytu Candida dubliniensis v klinickém materiálu a její identifikaci. Druhy rodu Candida spp. se vyskytují jako komenzálové na kůži, v ústech a gastrointestinálním traktu, mohou se však stát za určitých podmínek patogenními. Výskyt C. dubliniensis v nemocničním prostředí se zvyšuje a jednou z příčin by mohl být na základě selekční tlak nejčastěji používaného antimykotika flukonazolu. K identifikaci kmenů byla použita jednak latexová aglutinace pomocí soupravy Bichro-Dubli Fumouze, jako screeningová metoda, ale také metoda MALDI-TOF, která byla použita pro konečné potvrzení identifikace. U izolátů z oddělení JIP byly vypočítány hodnoty MIC mikrodiluční metodou SensiTitre YeastOne. Pro flukonazol byly stanoveny hodnoty MIC50, MIC90, GM (0,12 ?g/ml, 0,12 ?g/ml, 0,30 ?g/ml). V současné době tento jev pozorujeme nejen u pacientů s imunodeficitem. V Ostravě byl zaznamenán 9% výskyt izolátu. In vitro nebyly prokázány rezistence.

Abstract:

The diploma thesis is focused on monitoring the occurrence of Candida dubliniensis in clinical material and its identification. Candida spp. they occur as commensals on the skin, mouth and gastrointestinal tract, but may become pathogenic under certain conditions. The incidence of C. dubliniensis in the hospital environment is increasing, and one of the causes could be based on the selection pressure of the most commonly used fluconazole antifungal agent. Currently, this phenomenon is not only observed in immunodeficiency patients. In Ostrava was recorded 9% occurrence of the isolate. To identify the strains, latex agglutination using the Bichro-Dubli Fumouze kit was used, as a screening method, but also the MALDI-TOF method, which was used for final confirmation of the identification. For ICU isolates, MIC values were calculated using the SensiTitre YeastOne microdilution method. MIC 50, MIC 90, GM (0,12 ?g/ml, 0,12 ?g/ml, 0,30 ?g/ml) were determined for fluconazole. Currently, this phenomenon is not only observed in immunodeficiency patients. There was a 9% incidence of isolate in Ostrava. No resistance was demonstrated in vitro.
