Possibilities of combined treatment with vitamin D3 and all-trans retinoic acid in osteosarcoma cell lines

Abstract:

Protokol COMBAT je druh nízkodávkové metronomické chemoterapie, která představuje méně toxickou variantu léčby dětských nádorů. V současnosti je nabízen dětským pacientům s relabujícími solidními nádory v Dětské nemocnici v Brně. Léčebný režim je založen na kombinaci cytotoxických léčiv, antiangiogenních látek, induktorů diferenciace a dalších biologických látek, které podporují léčbu. V případě kombinované terapie je velmi důležité poznat mechanismy účinku jednotlivých léčiv, aby při jejich správné kombinaci došlo k zesílení terapeutického účinku, k snížení dávky a toxicity a k minimalizaci rezistence na léky. Předkládaná rigorózní práce je zaměřena na kombinaci dvou diferenciačních činidel zařazených v protokolu COMBAT: vitaminu D3 ve formě kalcidiolu a kalcitriolu a kyseliny all-trans retinové (ATRA). Hlavním cílem této práce bylo analyzovat možnou modulaci antineoplastického účinku vitaminu D3 kombinovanou aplikací s ATRA u sbírkové osteosarkomové buněčné linie Saos-2 a u šesti buněčných linií osteosarkomu derivováních z nádorové tkáně pacientů. Citlivost k diferenciačním činidlům aplikovaným samostatně byla velmi variabilní a specifická pro každou buněčnou linii, ale i přesto byl pozorován statisticky významně zesílený účinek kombinované léčby kalcitriolu s ATRA u téměř všech testovaných linií. V návaznosti na výsledky proliferačních testů byly analyzovány změny exprese jaderných receptorů pro vitamin D (VDR) a retinoidy (RARα, RXRα) po aplikaci samotných diferenciačních činidel. V případě buněčných linií s vysokou bazální hladinou RARα bylo detekováno zvyšování hladiny VDR po aplikaci ATRA. Na základě získaných výsledků byl popsán jeden z možných mechanismů interakce kalcitriolu s ATRA. Tento mechanismus je založen na změně konformace RARα po navázání ATRA, následkem čeho dojde k ukončení represe VDR. Podobný mechanismus byl již popsán u buněčných linií akutní promyelocytární leukemie a touto studií se potvrdil i na buněčných liniích osteosarkomu. Získané výsledky v předložené rigorózní práci dokazují, že kombinovaným působením kalcitriolu a ATRA lze dosáhnout zvýšeného antineoplastického účinku u osteosarkomových linií in vitro, přičemž hladina RARα v buňkách osteosarkomu by mohla sloužit jako prediktivní faktor pro možnou synergii těchto diferenciačních činidel.

Abstract:

COMBAT is a low-dose metronomic chemoterapeutical protocol currently used in the Children´s Hospital in Brno, which represents a less toxic regimen in the treatment of pediatric tumors. This protocol is based on the combination of cytotoxic drugs, anti-angiogenic agents, differentiation inducers and other biologicals. For this kind of combined therapy, it is very important to be familiar with the mechanism of action of individual drugs to achieve stronger therapeutic effect, reduce dosage and toxicity as well as minimize drug resistance. This thesis is focused on the combination of two differentiation inducers included in the COMBAT protocol: vitamin D3 in the form of calcitriol or calcidiol and all-trans retinoic acid (ATRA). The main aim of this study was to analyze a possible modulation of the antineoplastic effect of vitamin D3 by its combined application with ATRA in Saos-2 reference osteosarcoma cell line and in six patient-derived osteosarcoma cell lines. The response to treatment with differentiation inducers alone was very variable and specific for each cell line. Nevertheless, a significantly stronger effect of combined treatment with calcitriol and ATRA in the majority of tested cell lines was observed. Final part of study was focused on expression changes in nuclear receptors for selected differentiation inducers after treatment. Surprisingly, vitamin D receptor (VDR) up-regulation by ATRA in cell lines with high basal level of retinoic acid receptor α (RARα) was detected. Based on this result, one of the possible mechanisms of ATRA and calcitriol synergistic interaction was described. This mechanism is based on RARα conformation change after ATRA binding and the subsequent termination of VDR repression by RARα. This type of interaction has already been observed in acute promyelocytic leukemia cell lines and in this thesis was confirmed in osteosarcoma cells. In general, the obtained results suggest that the combination of calcitriol and ATRA can achieve enhanced anti-proliferative effect in vitro. Moreover, the level of RARα in osteosarcoma cells could serve as a predictive factor for a possible synergy of these two differentiation inducers.
