Health care providers' attitudes towards people with dementia

Abstract:

Péče o pacienty s demencí je velmi náročná a klade vysoké nároky na pečující. Cílem práce bylo zjistit, jak se k pacientům s demencí chovají zdravotníci a další profesionální pečující ve vybraných zařízeních a zda bude pozorován rozdíl v chování zdravotníků v zařízení se supervizí a bez supervize. K získání potřebných dat byla použita metoda přímého zúčastněného skrytého pozorování. Toto pozorování bylo doplněno polostrukturovanými rozhovory s vybranými pracovníky, zaměřenými na jejich motivaci k výkonu pomáhajícího povolání, zvládání obtížných situací a možné způsoby podpory v podobě vzdělávání, supervize a podpory od nadřízených. V obou zařízeních byl zaznamenán výskyt nevhodného až patologického chování zdravotníků k pacientům, byla zaznamenána nevhodná komunikace (elderspeak, výhružky atd.), nešetrná fyzická manipulace s pacienty i ignorování jejich potřeb ze strany pečujících. Z rozhovorů vyplynulo, že pracovníci, kteří nemají přistup k supervizi by tento způsob podpory uvítali a také to, že mnozí zdravotníci nepociťují podporu ze strany svých nadřízených.

Abstract:

Caring for patients with dementia is very demanding and places great demands on carers. The aim of this work was to find out how health professionals in selected health/social-care institu-tions behave to patients with dementia and whether there is a difference in the behaviour of health professionals in the institutions with and without supervision. The method of direct involved hidden observation was used to obtain the necessary data. This observation was sup-plemented by semi-structured interviews with selected staff, aimed at their motivation to pur-sue a helping profession, managing difficult situations and possible ways of supporting them in the form of education, supervision and support from their superiors. In both institutions, the incidence of inadequate to pathological behaviour of health professionals to patients was not-ed, inappropriate communication (elderspeak, threats, etc.), inconvenient physical manipula-tion with patients and ignoring their needs by carers. The interviews showed that workers who do not have access to supervision would welcome this type of support and that many health professionals do not feel the support of their superiors.
