Abstract:

Náplní klinické části disertační práce je zhodnocení úspěšnosti léčby chondrálních a osteochondrálních defektů kolenního kloubu s využitím modifikované AMIC (Autologous Matrix-Induced Chondrogenesis) techniky, při které jsou kombinovány mikrofraktury spodiny s implantací bezbuněčného implantátu na bázi kolagenu I. typu ve dvouletém horizontu. Jako kontrolní skupina slouží pacienti ošetření prostými mikrofrakturami spodiny defektu. Experimentální část je zaměřena na kvalitativní vyšetření chrupavky pomocí dGEMRIC (delayed Gadolinium-Enhanced MRI of Cartilage) sekvence s cílem stanovit obsah glykosaminoglykanových řetěžců v místě ošetření defektu, v okolí defektu a v referenčním místě u obou použitých metod. Do prospektivního sledování bylo zařazeno 15 pacientů ošetřených metodou modifikované AMIC techniky a 12 pacientů ošetřených mikrofrakturami. Klinické výsledky se sledovaly pomocí Knee injury and Osteoarthritis Outcome Score (KOOS), Lysholmova skóre a Tegnerovy škály aktivity předoperačně a následně v odstupu 6, 12 a 24 měsíců od operace. V pooperačním horizontu 6, 12 a 18 měsíců byla provedena kontrolní vyšetření pomocí magnetické rezonance. Ta zahrnovala jednak morfologické vyšetření chrupavky, která se hodnotila pomocí Magnetic Observation of Cartilage Repair Tissue (MOCART) skóre, a dále pak kvalitativní vyšetření chrupavky pomocí metody dGEMRIC, kdy byl na základě intravenózně podané kontrastní látky Gd - DTPA2- (gadolinium diethylen triamin pentaacetonová kyselina) a její distribuce v kloubní chrupavce stanovován obsah glykosaminoglykanů, vyjádřen dGi indexem. Na základě statistického zpracování výsledků klinických dotazníků a morfologického vyšetření magnetickou rezonancí pomocí MOCART skóre je možné modifikovanou AMIC techniku považovat za moderní, efektivní metodu v léčbě ohraničených chrupavčitých lézí kolenního kloubu. Při kvalitativním vyšetření chrupavky došlo ve sledovaném období 18 měsíců u většiny pacientů k poklesu obsahu GAG jak v místě ošetření defektu, tak v referenční, nepoškozené oblasti. K vyhodnocení změn kvality chrupavky u obou typů operace je velmi pravděpodobně nutný delší časový úsek. Souhrnně se dGEMRIC sekvence ukazuje jako jedna z možností vyšetření kvality chrupavky, která vhodně doplňuje informace získané morfologickými sekvencemi. Pro běžné použití v každodenní klinické praxi má však nevýhody v podobě dlouhé doby jednotlivých vyšetření, nutnosti intravenózního podání kontrastní látky a vysoké ceny vyšetření.

Abstract:

The core of the clinical part of this dissertation is to evaluate the success rate of chondral and osteochondral defects of the knee joint utilizing the modified AMIC (Autologous Matrix-Induced Chondrogenesis) technique. The technique combines microfructures of the underlying bone with the implantation of acellular type I collagen-based implant within a period of two years. As a control group patients treated with simple microfractures of the underlying bone of the defect were chosen. The experimental part focuses on the qualitative examination of cartilage utilizing the dGERMRIC (delayed Gadolinium-Enhanced MRI of Cartilage) sequences. The aim was to determine the volume of glycosaminoglycan chains in the site treated for the defect, around the defect and at the referential site for both methods used. Fifteen patients treated with the modified AMIC technique and twelve patients treated with microfractures were involved in the prospective follow-up. Clinical results were assessed using the Knee injury and Osteoarthritis Outcome Score (KOOS), the Lysholm score, and the Tegner Activity Scale, pre-op and at 6, 12 and 24 months post-op. Post-op follow-up MRI checks at 6, 12 and 18 months were performed. The MRI included both the morphological examination of the cartilage which was assessed by the Magnetic Observation of Cartilage Repair Tissue (MOCART) score, as well as the qualitative examination of the cartilage with the dGEMRIC method, where based on the intravenous application of the Gd-DTPA2- contrast media (gadolinium diethylenetriaminepentaacetate ) and its distribution in the knee cartilage the volume of glycosaminoglycans was determined, expressed by the dGi index. Based on the statistical processing of the clinical assessment questionnaire results and the morphological examination using MRI utilizing the MOCART score, the modified AMIC technique can be considered as a modern, effective treatment method of defined cartilage lesions in the knee joint. The qualitative examination of the cartilage shows in most patients a decrease of the GAG content both at the site of the defect treatment and in the reference, undamaged area during the monitored period of 18 months. Most probably, a longer period is required to be able to evaluate changes in the cartilage for both types of procedures. The dGEMRIC sequence shows to be one of the options to examine the quality of cartilage which conveniently complements the information obtained using morphology sequences. For general use in everyday clinical practice, however, there are disadvantages related to this technique, such as the length of each examination, the necessity of intravenous application of the contrast media and the costs of the examination.
