Difficulties in the Evaluation of Cervical Cytological Smears

Abstract:

Teoretická část bakalářské práce obsahuje základní shrnutí o anatomii a histologii ženského pohlavního systému včetně cytologické klasifikace buněk poševního epitelu. Je zde popsáno zpracování cytologického materiálu, hodnocení buněčného stěru a jeho následná klasifikace dle Bethesdy 2001. Non-neoplastické nálezy jsou rozděleny do tří skupin ? specifické záněty, reaktivní buněčné změny (spojené se zánětem, IUD a ozářením) a ostatní nespecifické změny. Krátce jsou zde zmíněny také intraepiteliální léze. Poslední kapitola objasňuje pojmy ? falešná pozitivita a falešná negativita. Na tyto dvě skupiny navazuje podkapitola shrnující faktory, které můžou ovlivnit cytologické hodnocení. Praktická část se zabývá vlastním pozorováním na souboru cytologických preparátů. Úkolem je vyhodnotit zcela jasné cytologické nálezy a klást důraz na nálezy s malými odlišnostmi, které mohou nasvědčovat pro falešnou pozitivitu a falešnou negativitu.

Abstract:

The theoretical part of this bachelor thesis contains basic summary about anatomy and histology of female genital system, including the cytological classification of cervical epithelium´s cells. This part also describes the elaboration of cytological material, evaluation of cellular smear and its consecutive classification according to Bethesda 2001. Non-neoplastic findings are divided in three groups- specific inflammation, reactive cellular changes (connected with inflammation, IUD and irradiation) and other nonspecific changes .The intraepithelial lesion is also shortly mentioned. The last chapter clarifies terms- false positivity and false negativity. The subchapter summarizing factors, which can influence cytological classification results from these two groups. The practical part of the thesis deals with observing the set of cytological preparative. The task is to evaluate absolutely distinct cytological findings and to insist on findings with very small differences which can testify to the false positivity and false negativity.
