Keratoconus - possibilities of treatment and correction

Abstract:

Abstrakt Cílem této práce bylo stručně popsat patologické změny na oku postiženým keratokonem, jak toto onemocnění ovlivňuje zobrazovací schopnosti oka. Největší část byla věnována možnostem korekce a jaké jsou možnosti léčby. Keratokonus je ektatické onemocnění rohovky, které se projevuje jejím kónickým vyklenováním a protenčováním. Tím vzniká myopie a astigmatismus. Výskyt je obvykle bilaterální, i když asymetrický mezi oběma očima. Odhaduje se, že se vyskytuje asi u 1 % populace. Keratokonus se objevuje obvykle v pubertálním věku a má tendenci progredovat až do třetí nebo čtvrté dekády života. V počátečních stádiích keratokonu bývá dostačující korekce dioptrickými brýlemi nebo měkkými kontaktními čočkami. Tvrdými kontaktními čočkami je možné korigovat vyšší stupně astigmatismu a nerovnosti na rohovce vyskytující se u pokročilejších stádií. Nesnáší-li pacient kontaktní čočky, mohou se do rohovky implantovat intrastromální rohovkové segmenty, které rohovku oploští. K zastavení progrese onemocnění se v počínajících stádiích indikuje Collagen Corneal Cross Linking, při kterém se působením UVA záření na rohovku nasycenou riboflavinem mění její biochemická struktura, a tím dochází k jejímu zpevňování. V případech, kdy nejsou kontaktní čočky schopny zajistit normální vidění, se přistupuje k operacím. Nejčastěji se provádí perforující keratoplastika nebo přední hluboká lamelární keratoplastika.

Abstract:

Abstract The objective of this thesis was briefly to describe pathological changes on the eye with keratoconus, and how this disease affects visual acuity of the eye. Most part of the thesis was dedicated to possibilities of correction and treatment. Keratoconus is an ecstatic disease of the cornea, manifesting with conic arch and thinning, which can lead to myopic and astigmatic progression. Occurrence of kerakotonus is almost bilateral and asymmetric between the two eyes and the presence in population is estimated at 1%. Keratoconus usually occurs at pubescent age and the progression stops in late thirties or early forties. In the early stages of keratoconus, there is adequate correction with spectacles or with a soft contact lens. With hard lens is possible to correct high astigmatism and ragged surface of the cornea, which is present at advanced stages of keratoconus. If a patient is intolerant to contact lenses, intrastromal corneal rings can be implanted to flatten the cornea. In the early stages is indicated Collagen Corneal Cross Linking in order to stop progression of the disease by UVA irritation on the cornea saturated with riboflavin, which is the way to harden the cornea by the biochemical reaction. Surgeries are indicated in cases, when contact lenses are useless because of inability to provide a normal vision. The most common surgery is penetrating keratoplasty or inferior lamellar keratoplasty.
