Incidence and risk factors of perineal injury of women during vaginal delivery

Abstract:

Cíl: Cílem práce bylo zjistit výskyt porodního poranění hráze (ruptury I. - III. stupně a episiotomie) za rok 2015 v Nemocnici Nový Jičín a.s. a ověřit, zda vybrané faktory mohou ovlivňovat výskyt těchto poranění. Metodika: K výzkumu byla použita kvantitativní metoda formou obsahové analýzy dokumentů. Výzkumný soubor tvořila zdravotnická dokumentace od 513 žen, které v roce 2015 porodily vaginálně v Nemocnici Nový Jičín a.s. Sběr dat probíhal v období od července 2016 do června 2017. Pro popis výzkumného souboru byla použita popisná statistika. Dále byl použit chí-kvadrát, Fisherův exaktní test a vícefaktorová analýza. Výsledky: Incidence episiotomie ve výzkumném souboru byla 57 %. Incidence spontánního porodního poranění hráze byla 31 %. Největším rizikem pro vznik spontánního porodního poranění hráze byl vyšší věk rodiček, zvláště vícerodiček. Druhým největším rizikovým faktorem byla vyšší porodní hmotnost novorozence. Mezi další rizikové faktory porodního poranění hráze patřila parita rodiček a délka druhé doby porodní. Protektivním faktorem bylo provedení episiotomie. Závěr: K provedení episiotomie by se nemělo přistupovat rutinně. Před každým provedením episiotomie je důležité pečlivě zvážit, zda je indikována. V opačném případě je chybou váhat s provedením episiotomie tam, kde indikace k provedení je. Neprovedení episiotomie může mít za následek rozsáhlá poranění na hrázi.

Abstract:

Aim: The aim of the study was to establish the incidence of injuries to the perineum during childbirth (1st - 3rd level ruptures and episiotomy) during 2015 at Nemocnice Nový Jičín a.s. and to verify whether some selected factors may affect the incidence of these injuries. Methodology: It was used a quantitative method to analyze of documents. Research involved 513 items of medical documentation, in this case the labour documentation of mothers who gave vaginal birth to newborns at Nemocnice Nový Jičín a.s. in 2015. Data was compiled during the period from July 2016 to June 2017. Descriptive statistics were used to describe the researched files. The Chi-squared test and Fisher's exact test were used to evaluate hypotheses and a multi-factor analysis was used in conclusion. Results: The incidence of episiotomies in the researched files was 57%. The incidence of spontaneous perineal injury was 31%. The greatest risk factor for spontaneous perineal injury during childbirth is greater maternal age (especially if multiparous). The second biggest risk factor was the higher birth weight of the newborn. Other risk factors for perineal injuries include maternal parity and the duration of the second stage of labour. Performance of an episiotomy was a protective factor. Conclusion: Performance of an episiotomy should not be routine. Before performing an episiotomy it is important to carefully consider whether this procedure is indicated. On the contrary, it is a mistake to hesitate to perform an episiotomy where indicated. Failure to perform an episiotomy could result in extensive perineal injury.
