Critical review of existing models of glomerular filtration rate and their applicability in clinical practice in oncology

Abstract:

Ke sledování různých hledisek zdravotního stavu onkologicky nemocných pacientů existuje mnoho metod a postupů. Tato práce se zabývá komplexní kritickou komparativní studií dostupných modelů predikujících stupeň glomerulární filtrace (nejen) u onkologicky nemocných pacientů, tzn. studuje, zkoumá a kriticky zhodnocuje Leveyho vzorce (MDRD vzorec dlouhý a MDRD vzorec krátký) a vzorec Cockroft-Gault v aplikaci v onkologické klinické praxi. Dále také sleduje efektivnost vzorců v odhadech stupně glomerulární filtrace a clearance kreatininu pacientů a specifické trendy jednotlivých aplikací a simulací predikčních modelů, které jsou rovněž součástí práce. Na podkladě těchto simulačních grafů pak lze vyvodit zmíněné závěry o míře efektivnosti modelů dle různých aspektů daných onkologickou klinickou praxí na různých úrovních (antropometrická a biochemická charakteristika pacientů, onkologická léčba…). Zjišťování stupně glomerulární filtrace je velmi důležité pro odhad clearance kreatininu u onkologických pacientů, a tedy pro zjištění celkového zdravotního stavu pacientů. Výborná znalost zdravotního stavu pacientova organismu posléze umožňuje lékařům co nejefektivněji podávat pacientům různé formy onkologické léčby, zejména pak chemoterapie. Práce je založena na požadavku moderní onkologie zkoumat celou problematiku clearance kreatininu v co nejkomplexnějším měřítku. Výsledky celé studie jsou porovnány s reprezentativními daty poskytnutými Masarykovým onkologickým ústavem (MOÚ). Součástí studie jsou pojednání o významu kreatininu a metabolických pochodech s ním spjatých jako o jedné z nejdůležitějších fyziologických látek organismu, která má nenahraditelnou výpověďní hodnotu o celkovém zdravotním stavu pacienta. Dále se práce zabývá tématikou funkce ledvin v organismu, onemocněním ledvin, diferenciální diagnostikou selhávání ledvin, chemoterapeutickou léčbou a faktory, které ji ovlivňují. Tato bakalářská práce je podkladem pro práci diplomovou.

Abstract:

There are a lot of methods and procedures to follow different aspects of health status of cancer patients. This work deals with complex comparative critical study of available regress models to estimate glomerular filtration rate (not only) of cancer patients, especially focused on Leveys formulas (long MDRD equation and short MDRD equation) and Cockrofs & Gaults formula application to clinical practice. It also observes the effectivity of the mentioned formulas in estimating of glomerular filtration rate and creatinine clearance of patients, specific trends in particular applications and model simulations. These are a part of this study and it is possible to deduct conclusions about their effectiveness in different aspects of oncological clinical practice at various levels (e.g. anthropometric and biochemical characteristic of patients, oncologic treatment, etc.). The glomerular filtration rate is the most important aspect for estimating creatinine clearance of cancer patients and therefore to detect the overall health status of patients. Perfect knowledge of creatinine clearance of patients allows doctors to use the most effective method to treat their patients (e.g. chemotherapy dosages) This study is based on modern medicine's need to examine the whole creatinine clearance issue in the most complex criterion. The results of the whole study are compared with consecutive data from Masaryk Memorial Cancer Institute. Another part of the study is discourse on importance of creatinine and metabolical processes linked with it as one of the most important physiological substances of organism because it is irreplaceable in determination of patient's overall health status. Furthermore, the work deals with the issues of kidney function in the body, kidney diseases, differential diagnosis of renal failures, chemotherapy treatments and factors that affect them. This bachelor´s work is a foundation for a master´s thesis.
