New Molecules in the Pathophysiology of Ventilatory Control in Heart Failure Patients

Abstract:

U pacientů se srdečním selháním koncentrace leptinu inverzně koreluje s koncentrací natriuretických peptidů. Nízká koncentrace leptinu je asociována s přítomností centrální spánkové apnoe a jak nízká tak i vysoká koncentrace leptinu je asociována s hyperventilací (zvýšené VE/VCO2 a nízké PETCO2) v klidu a během zátěže. Příčinou této nelineární závislosti (tvar písmene U) by mohl být buď absolutní nedostatek leptinu, nebo leptinová rezistence v případě vysoké koncentrace. Vyšší koncentrace leptinu u žen se srdečním selháním by mohla působit protektivně proti vzniku hyperventilace a CSA. Kontraregulační vztah leptinu a natriuretických peptidů by mohl mít význam v patofyziologii kontroly ventilace u pacientů se srdečním selháním.

Abstract:

In HF patients circulating leptin concentration is inversely correlated with natriuretic peptides concentration, suggesting a counter-regulatory relationship. Increased circulating leptin levels may also contribute to the low BNP concentration observed in HF patients with high BMI and may promote the obesity paradox of HF. Low leptin levels are associated with the presence of CSA in HF patients and both high and low leptin concentrations are associated with hyperventilation (increased VE/VCO2 and decreased PETCO2) at rest and during exercise. This non-linear “U-shaped” relation may be caused by either leptin deficiency or leptin resistance that may modulate ventilatory control. Higher levels of circulating leptin might protect women from HF associated hyperventilation and development of CSA. Future studies aimed at investigation of leptin and natriuretic peptide interactions may be important for further understanding of the pathophysiology of CSA and ventilatory control in HF patients.
