The effect of kinesiotherapy in patients with Parkinson´s desease on stability and gait

Abstract:

Cílem diplomové práce bylo pomocí klinických testů zhodnotit vliv kinezioterapie na stabilitu a chůzi u pacientů s Parkinsonovou nemocí (PN). Dílčí cíl se zaměřoval na zhodnocení vlivu kinezioterapie na celkovou kvalitu života a porovnání účinku kinezioterapie a účinku terapie ve virtuální realitě (VR). Diplomová práce byla zpracována v rámci projektu, ve kterém se sledovaly dvě skupiny pacientů. Jedna skupina absolvovala konvenční rehabilitaci a druhá skupina rehabilitaci s využitím VR. Výzkum předkládané diplomové práce byl zaměřen především na skupinu s konvenční rehabilitací. Skupinu s konvenční rehabilitací tvořilo 9 pacientů (věk 70,2 ? 7,6 let) a skupinu s VR tvořilo 9 pacientů (věk 70, ? 5,9 let), kteří splnili vstupní kritéria. Vstupními kritérii bylo: pacient ve věku nad 18 let, spolupracující, samostatně mobilní, ve stabilizovaném stavu s diagnózou PN ve stádiu 1-3 podle dle Hoehnové a Yahra. Konvenční rehabilitace probíhala 2x týdně 60 minut po dobu 4 týdnů a terapie ve VR probíhala 2x týdně 20 minut po dobu 4 týdnů. K hodnocení účinku terapie byly použity klinické testy: Berg Balance Scale, Timed Up and Go, Timed Up and Go + dual task, Desetimetrový test chůze (rychlé i pomalé) a standardizovaný dotazník kvality života Parkinson´s Disease Questionnaire. Výsledky výzkumu poukazují, že u sledované skupiny pacientů s konvenční terapií došlo po terapii ke zlepšení stability, chůze i kvality života. Ke zlepšení došlo také po terapii ve VR u všech sledovaných parametrů. Při vzájemném porovnání obou skupin bylo zjištěno, že v testování chůze došlo k většímu zlepšení po terapii ve VR, ale stabilita a celkové skóre kvality života se více zlepšilo u pacientů po konvenční terapii. Z výsledků výzkumu vyplývá, že fyzioterapie je důležitou součástí komplexní léčby PN.

Abstract:

The aim of the thesis was to evaluate the effect of kinesiotherapy on stability and gait in patients with Parkinson's disease (PD) using clinical tests. The sub-objective was to evaluate the impact of kinesiotherapy on overall quality of life and to compare its effectivness to the effect of virtual reality (VR) therapy. The thesis was developed in the framework of a project in which two groups of patients were monitored. One group received conventional rehabilitation, while the other group received rehabilitation using VR. The research in the present thesis mainly focused on the group that received conventional rehabilitation. The conventional rehabilitation group consisted of 9 patients (age 70.2 ? 7.6 years) and the VR group consisted of 9 patients (age 70, ? 5.9 years) who met the entry criteria. The entry criteria were: patient aged over 18 years, cooperative, independently mobile, in stable condition with a diagnosis of PD stage 1-3 according to Hoehn and Yahr. Conventional rehabilitation was performed twice a week for 60 minutes over 4 weeks while VR therapy was performed twice a week for 20 minutes over 4 weeks. Clinical tests used to assess the effect of therapy were Berg Balance Scale, Timed Up and Go, Timed Up and Go + dual task, Ten-meter walk test (fast and slow) and standardized Parkinson's Disease Quality of Life Questionnaire. The results of the research indicate that patients with PD who received conventional therapy showed improvements in stability, gait and quality of life after therapy. Improvement also occurred after VR therapy in all parameters studied. When the two groups were compared with one another, it showed a grater improvement in gait testinkg after using VR therapy, whereas stability and overall quality of life score was more improved in patients after receiving conventional therapy. The results of this research suggest that physiotherapy is an essential component of the comprehensive treatment of PD.
