The comparison of patient´s lifestyles based on taking selected anticoagulants

Abstract:

Úvod: Antikoagulancia jsou léky regulující srážlivost krve, tato medikace je užívaná u pacientů s kardiovaskulárním onemocněním (onemocněním srdce a cév). U těchto chorob je životní styl jedním z hlavních ovlivnitelných rizikových faktorů. Metodika: K realizaci výzkumu byla pro sběr dat zvolena metoda kvantitativní analýzy formou standardizovaného dotazníku. Data byla získána od pacientů z kardiologických ambulancí Benedor v Ostravě. Sběr dat probíhal v rámci pilotní studie "Životní styl, kvalita života a compliance u pacientů s antikoagulační léčbou" v rámci projektu Studentské grantové soutěže. Výběrovým kritériem pro zařazení do pilotní studie bylo zvoleno užívání warfarinu, dabigatranu, rivaroxabanu nebo apixabanu, a současně podpis informovaného souhlasu pacienta. Data byla zpracována v programu Epidata a Microsoft Office Excel 2010. Hypotézy byly testovány chíkvadrát testem, a hladina významnosti byla nastavena na 5%. Výsledky: Výzkumný vzorek tvořilo 39 pacientů, převládal u nich důchodový věk nad 60 let. Věkovou kategorii do 60 let představovali pouze 2 pacienti. Nejčastějším užívaným antikoagulanciem byl warfarin. Téměř polovina pacientů (49 %) trpěla nadváhou (dle parametru Body Mass Index - BMI). Nejčastější komorbidita byla hypertenze. U rizikového faktoru kouření bylo zjištěno, že pouze jeden pacient (2 %) ze vzorku je aktivní kuřák. Co se fyzické aktivity týká, nejvíce pacienti preferovali chůzi nebo středně těžkou fyzickou aktivitu 22 pacientů (56 %), žádnou fyzickou aktivitu nevykonávalo 12 pacientů (31 %) a převážně sedavou aktivitu nebo stání zvolilo 5 pacientů (13 %). Více než polovina 22 pacientů (57 %) byla edukována v oblasti životního stylu zdravotním personálem a dokázala rady začlenit do života, edukováno nebylo 11 pacientů (28 %) a 6 pacientů (15 %) po edukaci nebylo schopno rady do běžného života začlenit. Závěr: Na základě výsledků je možné navrhovat zejména preventivní opatření, která mohou zabránit progresi nemoci či přítomnost častých komorbidit, případně zlepšit celkové zdraví pacienta a tím i kvalitu jeho života.

Abstract:

Introduction: Anticoagulants are medication regulating blood clotting, this medication is used in patients with cardiovascular disease (disease of the heart and vessels). In these diseases, lifestyle is one of the main impressionable risk factors. Methodology: To implement the research and collect the data, a qualitative analysis via standardized questionnaire was used. The data was acquired from patients in Benedor cardiological ambulances in Ostrava, Eastern Bohemia. Data collection was carried out as a part of the ´Lifestyle, quality of life and compliance in patients with anticoagulant therapy´ pilot study supporting the project of Student grant competition. One selection criterion for being included into the pilot study was using Warfarin, Dabigatran, Rivaroxaban or Apixaban, and at the same time, the patient´s signature of informed consent.The data was processed using the Epidata and Microsoft Office Excel 2010 programmes. Hypotheses were tested using the chi square test, and level of importance was set at 5%. Results: The research file consisted of 39 patients with the prevailing age of over 60. The younger age category (up to 60 years of age) was represented by two patients only. The most frequently used anticoagulant medicine was Warfarin. Almost half of the patients (49%) suffered from overweight (as given by the Body Mass Index -BMI parameters). Hypertension made up the most frequent co-morbidity. In the risk factor of smoking, it was discovered that only one patient (2%) out of the sample is an active smoker. Concerning physical activity, the ones mostly preferred by the patients were walking or moderate physical activity - preferred by 22 (56%) patients, no physical activity was carried out by 12 (31%) patients, and mostly sedentary activity or standing up was chosen by 5 (13%) patients. More than a half out of 22 (57%) patients were educated in the area of lifestyle by medical staff, and were able to incorporate that advice into their life, 11 (28%) patients were not educated, and 6 (15%) patients having been educated, were not able to incorporate that advice into their every day life. Conclusion: Based on the results, it is possible to suggest mainly preventive measures that may prevent the progreassion of disease or presence of frequent co-morbidities, or perhaps improve the patient´s general health, and thus also the quality of their life.
