Abstract:

Úvod: Karcinom plic je nejčastějším zhoubným nádorovým onemocněním u mužů jak v České republice, tak i po celém světě. Každoročně je v ČR diagnostikováno přibližně 6 000 nových případů tohoto onemocnění, většinou ve třetím a čtvrtém stadiu onemocnění. V teoretické části diplomové práce je popsána epidemiologická charakteristika a možnosti prevence karcinomu plic. Cílem praktická část bylo zjistit na základě dotazníkového šetření úroveň informovanosti o karcinomu plic a možnosti prevence tohoto onemocnění u souboru zdravotníků a nezdravotníků. Metodika: Soubor tvořily skupiny - zdravotníci (N = 99) a nezdravotníci (N = 100). Dotazník obsahoval informativní a znalostní otázky. Správná odpověď u znalostních otázek byla hodnocena jedním bodem (max. počet bodů 18). Pro hodnocení dat byla použita základní popisná statistika (aritmetický průměr, směrodatná odchylka, frekvenční tabulky). Pro testování rozdílů mezi skupinami byl použit dvouvýběrový t-test, ?2-test, Fisherův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Pro vytipování populačních skupin s nízkou znalostí byla použita metoda rozhodovacích stromů. Výsledky: Skupiny se významně lišily ve věku, pohlaví a vzdělání. Procento správných odpovědí se pohybovalo od 60,0 % až 76,4 %. Nejlepší výsledky byly dosaženy u otázek týkajících se příznaků karcinomu plic, úspěšnosti léčby a prevence. Z rizikových faktorů bylo největší procento správných odpovědí u faktoru kouření (95 %). Významně větší podíl správných odpovědí u zdravotníků proti nezdravotníkům byl u otázek týkajících se věkové skupiny nejvíce postižené karcinomem plic, příznaků, prevence a rizikových faktorů karcinomu plic. Mezi skupinami byl zjištěn statisticky významný rozdíl v celkové úrovni znalostí. Průměrný počet bodů u zdravotníků byl 8,5 bodů, u nezdravotníků 7,2 body. Ve skupině s nejmenším počtem bodů převládali respondenti se základním a středoškolským vzděláním bez maturity. U více než poloviny respondentů převládal názor, že informovanost o prevenci karcinomu plic není dostatečná. Informace o karcinomu plic čerpají nezdravotníci především ze sdělovacích prostředků, zdravotníci z odborné literatury. Téměř 60 % z celého souboru navštěvuje pravidelně preventivní prohlídky. Závěr: Z dotazníkového šetření vyplývá, že i když se prevenci karcinomu plic věnuje poměrně velká pozornost, informovanost neodborné veřejnosti, hlavně v oblasti rizikových faktorů karcinomu plic, je stále nedostatečná.

Abstract:

Introduction: Lung cancer is the most frequent malignant tumour disease in men in the Czech Republic and also worldwide. Every year there is about 6 000 new cases of the disease diagnosed in the Czech Republic, mostly in the third and fourth stage of the disease. The theoretical part of this thesis describes the epidemiological characteristic and prevention possibilities of lung cancer. The aim of the practical part based on the questionnaire study in a group of people working in medicine and nursing (medics), and laymen (non-medics) was to determine the level of awareness of lung cancer and possibilities of disease prevention Methods: The sample included two groups ? medics (N = 99) and non-medics (N = 100). The questionnaire contained informative and knowledge questions. The correct answer in the knowledge questions was rated by one point (maximum score 18). Descriptive statistics (mean, standard deviation and frequency tables) was used for evaluation To test differences between groups were used two-sample t-test, ?2-test and Fisher´s exact test. Statistical analysis was performed using two-sample t-test, ?2 test, and Fisher exact test. Statistical tests were evaluated at a significance level of 5 %. For the identification of groups with low knowledge the method of decision tree was used. Results: The groups differed significantly by age, gender and education. The percentage of correct answers ranged from 60.0% to 76.4%. The best results were achieved in questions relating to lung cancer symptoms, treatment success and prevention. The largest percentage of correct answers on risk factors concerned smoking (95%). Major proportion of correct answers about the age group most affected by lung cancer, symptoms, prevention and risk factors of lung cancer was found in a group of medics. There were significant differences in the overall level of knowledge between groups of medics and non-medics. The average score of medics was 8.5 and 7.2 of non-medics. In the group with the lowest score the respondents with primary and secondary education without graduation dominated. More than half of respondents thought that the awareness of lung cancer prevention is insufficient. Non-medics get the information about lung cancer primarily from media, medics from scientific literature. Almost 60% of the whole sample attends preventive medical examinations regularly. Conclusion: Although the society pays attention at lung cancer prevention the survey shows that awareness of lay public is still insufficient, especially in lung cancer risk factors.
