Suspension trauma and patient rescue procedure

Abstract:

Tato práce se zaměřuje na fenomén traumatu z visu a postupem k zajištění pacienta, jež může vzniknout u osob, které nehybně visí v jakémkoliv postroji nebo laně. Teoretická část práce podrobně analyzuje koncept syndromu z visu, jeho etiologii a fyziologické mechanismy. Dále zkoumá možné důsledky tohoto stavu pro postižené jedince a ideální postup k zajištění pacienta dle nejnovějších dostupných postupů. Praktická část studie je založena na kvantitativním výzkumu, který zkoumá, jestli složky integrovaného záchranného systému (IZS) mají znalosti a postupují podle jednotného postupu, který je aktualizovaný o nejnovější doporučení. Dále zjišťujeme, jestli jsou zaměstnanci pravidelně proškoleni ohledně dané problematiky, jaká je incidence syndromu z visu a jaký je klinický obraz pacienta během visu. Šetření je provedeno anonymním dotazníkem, který je rozeslán mezi vybrané složky IZS, které mají určité zaměření, jedná se o lezce Hasičského záchranného sboru (HZS) Praha, Leteckou záchrannou službu (LZS) Zdravotnické záchranné služby Ústeckého kraje (ZZS ÚK) a Horskou službu (HS) Šumava. Provedeným výzkumem bylo zjištěno, že Složky IZS znají hlavní rizika a pojem syndrom z visu ale nemají jednotný postup k zajištění pacienta. Lezci HZS Praha postupují dle zastaralých doporučení. Školení zaměstnanců probíhá v nepravidelných intervalech, jediná HS je školena pravidelně. Incidence syndromu z visu u složek IZS je nízká.

Abstract:

This thesis focuses on the phenomenon of suspension trauma and the procedure for securing patients who may develop it while suspended pasive in any harness or on a rope. The theoretical part of the thesis extensively analyzes the concept of suspension trauma, its etiology, and physiological mechanisms. It also examines the possible consequences of this condition for affected individuals and the ideal procedure for patient management according to the latest available protocols. The practical part of the study is based on quantitative research, which investigates whether components of the integrated rescue system (IZS) have knowledge and adhere to a unified procedure updated with the latest recommendations. Additionally, it explores whether employees are regularly trained on the relevant issues, how many individuals have encountered suspension trauma interventions, and what the clinical picture of the patient is during hang. The survey is conducted through an anonymous questionnaire distributed among selected components of the IZS with specific focuses, including climbers of the Fire Rescue Service (HZS) Prague, the Air Rescue Service (LZS) of the Medical Rescue Service of the Ústí Region (ZZS ÚK), and the Mountain Rescue Service (HS) Šumava. The research found that components of the IZS are aware of the main risks and the concept of suspension trauma but do not have a unified procedure for patient management. The climbers of the HZS Prague follow outdated recommendations. Instruction occurs at irregular intervals, with only the HS being regularly trained. The incidence of suspension trauma syndrome is low.
