Functional changes after tendon transfer in patients after cervical spinal cord injury - OT view

Abstract:

Bakalářská práce je zaměřená na rekonstrukční chirurgii horních končetin u tetraplegiků metodou šlachového transferu z ergoterapeutického pohledu. V teoretické části jsou zmíněny anatomické znalosti týkající se horní končetiny, úchopová funkce horní končetiny, krční míšní léze, jak vypadají a fungují horní končetiny dle výšky cervikální léze a jaké jsou jednotlivé fáze léčby u tetraplegických pacientů. Největší část je zaměřená na šlachové transfery a typy zákroků. Praktická část obsahuje dotazník spokojenosti a Jebsen Taylor Hand Function Test, který hodnotí funkci HK a její změny po šlachovém ternaferu. K výzkumu se přihlásilo 16 klientů z celkového počtu 36 odoperovaných tetraplegiků na území Čerské republiky. Jednalo se o 15 mužů a 1 ženu, kteří podstoupili alespoň jeden rekonstrukční zákrok metodou šlachového transferu. Praktická část dále obsahuje kazuistiku a celý průběh procesu (od předoperačního vyšetření, provedení zákroku až po samotnou pooperační rehabilitační péči). Předoperační a pooperační období je zkoumáno zejména z hlediska ergoterapie. Veškerá vyšetření probíhala v Centrum Paraple v Praze. Hlavním cílem bylo zjistit, jestli tato metoda má u tetraplegiků smysl a zda opravdu dochází k pozitivním funkčním změnám. Také, jsou-li klienti spokojeni s provedeným výkonem a zda došlo ke zvýšení jejich kvality života. Na základě výsledků bylo zjištěno, že spokojenost a funkční změny mají vysokou úroveň. Na druhou stranu, z výsledků vyplývají určité nedostatky, týkající se používaných testů. Bylo by vhodní, kdyby v dalších výzkumech došlo k jejich částečným modifikacím. Dále byla zjištěna nedostatečná informovanost klientů a mezery v komunikaci mezi jednotlivými specialisty.

Abstract:

The bachelor thesis is focused on the occupational therapy view in the reconstructive surgery upper limb in tetraplegics by the method of tendon transfer. In the theoretical part are mentioned the anatomical knowledge concerning the upper limb, grip function of the upper limb, cervical spinal lesions, how the upper limbs look and work according to a level of the cervical lesion and what are the individual phases of treatment in the tetraplegic patients. The largest part is focused on the analysis of the tendon transfer and types of surgery. The practical part contains a questionnaire of satisfaction and Jebsen Taylor Hand Function Test, which evaluates the function of the upper limb and its changes after tendon transfer. 16 clients from the total number of 36 tetraplegics who was surgeried on the territory of the Czech Republic applied for the research. There were 15 men and 1 woman who underwent at least one reconstructive procedure using the tendon transfer method. The practical part also contains a case report and the whole process of the process (from the pre-operative, the operation to the post-operative rehabilitation care). The pre-operative and post-operative period is examined mainly in terms of occupational therapy. All examinations took place in the Centrum Paraple in Prague. The main aim was to find out whether this method has a meaning for tetraplegics and whether there are really positive functional changes. Also, whether clients are satisfied with their performance and their quality of life has increased. As far as the results of this work are concerned, it has been found that the satisfaction and functional changes are high. On the other hand, partial certain shortcomings result from partial results tasks. Since this work is being drafted on the basis of a pilot study, which is focused, as well as this bachelor thesis, on the issue of tendon transfer in tetraplegics in the Czech Republic, testing will be modified in further research. Other shortcomings are visible in the overall informability of clients and the communication barrier between individual specialists.
