The rehabilitation protocol for the resumption of sports activity in an adult patient with a fracture of the base of the fifth metatarsal: clinical case

Abstract:

A metatarsal bone or metatarsal is one of the five long bones present between the proximal phalanges and the tarsal bones of each finger. The foot has a total of twenty-six bones which seven are tarsals, five are metatarsals and fourteen phalanges. Each metatarsal is made up of three parts: the body, the base and the head. The base of the metatarsus is the proximal end, it articulates and borders on a bone of the tarsus. It is its proximal end, it precedes the body and the head, and it adjoins with one or more bones of the tarsus. It has the shape of a wedge and it has a rough area on both the palmar side and the dorsal side, important for engaging important ligaments of the foot. In this thesis will be discussed the clinical case of a 55 years old patient who reported pain at the base of the fifth metatarsal bone of the left foot and at the apex of the peroneal malleolus and, therefore, she has undergone a rehabilitation program consisted of strengthening the muscles with progressive load through exercises of kinesitherapy in the gym for the stability, for the balance and for the proprioception and hydrokinesitherapy in the pool taking advantage of the lack of gravity and resistance of water, as well as the use of high-power laser therapy for fifteen minutes in the entire painful area and passive joint mobilization performed by a physiotherapist. The goal of the thesis will be to reduce pain, recover the joint width and the walking pattern.

Abstract:

Un osso metatarsale o metatarso ? una delle cinque ossa lunghe presenti tra le falangi prossimali e le ossa del tarso di ciascun dito. Il piede ha in totale ventisei ossa di cui sette sono tarsali, cinque sono metatarsali e quattordici falangi. Ogni metatarso ? formato da tre parti: il corpo, la base e la testa. La base del metatarso ? l'estremit? prossimale, si articola e confina con un osso del tarso. Esso ? la sua estremit? prossimale, precede il corpo e la testa e confina con una o pi? ossa del tarso. Ha la forma di un cuneo e presenta un'area ruvida sia sul lato palmare che sul lato dorsale, importante per agganciare importanti legamenti del piede. All'interno di questo elaborato sar? descritto il caso clinico di una paziente di 55 anni che ha riferito dolore alla base del quinto osso metatarsale del piede sinistro e nell'apice del malleolo peroneale e, pertanto, ? stata sottoposta a un programma di riabilitazione consistito nel rinforzo dei muscoli con carico progressivo attraverso esercizi di chinesiterapia in palestra per la stabilit?, per l'equilibrio e per la propriocezione, e di idrochinesiterapia in piscina sfruttando la mancanza di forza di gravit? e la resistenza dell'acqua, nonché l'utilizzo della laserterapia ad alta potenza per 15 minuti in loco dolente e mobilizzazione articolare passiva eseguita da un fisioterapista. L'obiettivo della tesi sar? quello di ridurre il dolore, recuperare l'ampiezza articolare e lo schema del passo.
