Lifelong effects of extreme stress on the brain: Studying cortico-subcortical relationships using advanced magnetic resonance imaging techniques

Anotace:

Tato disertační práce zkoumá vliv dlouhodobého extrémního stresu na osoby přeživší holokaust a na následující dvě generace potomků přeživších. Pozornost je věnována především změnám ve strukturách a funkcích mozku, psychiky a také v některých genetických charakteristikách. Teoretická část práce popisuje oblasti mozku zranitelné vůči působkům stresu, vývojová období, kdy je mozek náchylnější k poškození a dopady stresu v podobě duševních onemocnění. Dále jsou přiblíženy výsledky některých prací, které zkoumaly následky holokaustu na zdraví a psychiku přeživších v průběhu posledních 70 let. Závěr teoretické části se věnuje možným způsobům transgeneračního přenosu stresu na potomky. V empirické části jsou zkoumány rozdíly v objemu šedé hmoty pomocí voxel-based morfomery a změny ve funkční konektivitě u přeživších holocaust oproti kontrolní skupině, se speciální pozorností k věku, ve kterém došlo k působní traumatického stresu. Dále jsou posuzovány přetrvávající psychologické symptomy posttraumatického stresu a posttraumatickému růstu, satisfakce s životem a úroveň wellbeingu v souvislosti s vnímáním národní identity, napříč všemi generacemi zasaženými holokaustem. Genetické studie hodnotí relativní délku telomer a počet kopií mitochondriální DNA. Závěr práce poukazuje na celoživotní nevratné přetrvávání následků stresu spojeného s holokaustem jak v mozku, tak psychice přeživších. Některé dopady stresu jsou předávány i do dalších generací. Práce zároveň zdůrazňuje důležitost věku, ve kterém je stres prožíván.

Abstract:

This dissertation thesis examines the impact of long-term extreme stress on Holocaust survivors and the next two generations of survivors' descendants. Attention is paid in particular to changes in the structures and functions of the brain and psyche, as well as in some genetic characteristics. The theoretical part of the work describes the areas of the brain vulnerable to the effects of stress, the developmental periods when the brain is more susceptible to damage and the consequences of stress in the form of mental illnesses. It also provides an overview of the results of work that has examined the impact of the Holocaust on the health and psyche of survivors over the past 70 years. The end of the theoretical section discusses possible ways transgenerational stress is transmitted to offspring. In the empirical part, differences in grey matter volume using voxel-based morphometry and changes in functional connectivity in Holocaust survivors versus a control group are examined, with particular attention to the age at which the stress exposure occurred. In addition, persistent psychological symptoms of posttraumatic stress and posttraumatic growth, satisfaction with life, and level of wellbeing are assessed concerning perceptions of national identity, across all HS generations. The last part of the thesis evaluates relative telomere lengths and mitochondrial DNA copy number. The conclusion of the thesis points to the lifelong irreversible persistence of the effects of Holocaust-related stress in both the brain and psyche of survivors and the fact that some of the effects of stress are transmitted to subsequent generations. The work also highlights the importance of the age at which stress is experienced.
