Changes in refraction after penetrating keratoplasty - possible solutions

Abstract:

Introduction. Corneal transplantation is the most common and successful human transplantation. High astigmatism is common refractive complication after otherwise successful corneal transplantation. Ametropia decreases functional result of penetrating keratoplasty (PK) in patients with transparent corneal graft. Presented thesis focused on evaluation of visual acuity and astigmatism in patients after LASIK for postkeratoplasty astigmatism in long-term follow-up. Methods. Retrospective noncomparative longitudinal analysis of 25 eyes (21 patients), who underwent laser in situ keratomileusis (LASIK) to correct postkeratoplasty astigmatism at the Department of Ophthalmology, The University Hospital Brno and the Medical School of the Masaryk University in Brno, Czech Republic between 01.01.2000 and 31.12.2010. We measured uncorrected and best-corrected visual acuity (UCVA, BCVA) in decimal scale with a Snellen´s visual acuity chart, manifest and topographical astigmatism (D) and spherical equivalent (D) after 6 months and then once per year. We used software IBM Statistics SPSS version 19 for statistics. Results. Our group consists of 25 eyes of 21 patients with average age at the time of LASIK surgery 37.3 ± 11.1 years. Average follow-up period was 12.8 ± 3.2 years (7 – 18 years). Repeated LASIK was necessary in 4 patients due to residual myopia and astigmatism. Average UCVA was 0.12 ± 0.09 preoperatively and improved significantly after 6 months postoperatively (0.48 ± 0.26, p < 0.0001). Improvement remained stable and significant also in 7 (0.42 ± 0.25, p < 0.0001), 10 (0.39 ± 0.26, p < 0.0001) and 15 years (0.24 ± 0.17, p = 0.0209) after LASIK. Average BCVA improved slightly after the surgery, but not significantly. Average spherical equivalent (SE) decreased significantly from -4.95 ± 3.78 D to -1.85 ± 2.16 D at 6 months (p = 0.0013), but decrease was not significant in the follow-up. Manifest astigmatism (MA) decreased significantly and permanently from -4.89 ± 1.71 D to -1.64 ± 2.27 D (p < 0.0001) after 6 months and to -2.26 ± 2.77 D (p = 0.0004), -2.88 ± 2.42 D (p = 0.0061), and -2.40 ± 1.19 D (p = 0.0045) at 7, 10 and 15 years postoperatively. Topographical astigmatism (TA) decreased slightly from 5.30 ± 2.05 D to 3.78 ± 2.03 D (p = 0.017) after 6 months, but this was not significant at 7, 10 and 15 years postoperatively. Conclusion. LASIK significantly improved UCVA, SE, TA and MA in our patients with postkeratoplasty astigmatism 6 months postoperatively. UCVA and MA remained significantly improved after 15 years.

Abstract:

Úvod. Transplantácia rohovky je najčastejšou a najúspešnejšou humánnou transplantáciou. Vysoký astigmatizmus predstavuje častú refrakčnú komplikáciu inak úspešnej transplantácie rohovky. Znižuje funkčný výsledok perforačnej keratoplastiky (PKP) u pacientov s čírym transplantátom. Dizertačná práca prináša dlhodobé sledovanie pacientov po PKP, ktorý podstúpili refrakčnú operáciu LASIK (laser in-situ keratomileusis) ku korekcii ametropie. Metodika. Retrospektívna nekomparatívna longitudinálna analýza 25 očí (21 pacientov), ktorý podstúpili LASIK po PKP na Očnej Klinike FN Brno a LF Masarykovej Univerzity medzi 01.01.2000 a 31.12.2010. U každého pacienta bola vyšetrená nekorigovaná a najlepšie korigovaná zraková ostrosť na Snellenových optotypoch, manifestný a topografický astigmatizmus a sférický ekvivalent 6 mesiacov po operácii a následne jeden krát ročne. Pri vybraných kontrolách prebehlo meranie počtu endotelových buniek. Na štatistické spracovanie bol použitý softvér IBM Statistics SPSS verzia 19 a program GraphPad. Výsledky. Súbor tvorilo 25 očí (21 pacientov) s priemerným vekom v čase operácie LASIK 37,33 ± 11,1 rokov. Priemerná dĺžka sledovacieho obdobia bola 12,8 ± 3,2 rokov (7 – 18 rokov). Štyria pacienti podstúpili LASIK opakovane pre reziduálnu refrakčnú vadu. Priemerná nekorigovaná zraková ostrosť (uncorrected visual acuity, UCVA) bola pred operáciou 0,12 ± 0,09 a po 6 mesiacoch sa signifikantne zlepšila na 0,48 ± 0,26 (p < 0,0001). Zlepšenie UCVA zostalo významné a stabilné do konca sledovacieho obdobia (v 7. roku 0,42 ± 0,25, p < 0,0001, v 10. roku 0,39 ± 0,26, p < 0,0001, v 15. roku 0,24 ± 0,17, p = 0,0209). Najlepšie korigovaná zraková ostrosť (best-corrected visual acuity, BCVA) sa po operácii zlepšila mierne, ale nesignifikantne. Priemerný sférický ekvivalent významne klesol po 6 mesiacoch z -4,95 ± 3,78 D na -1,85 ± 2,16 D (p = 0,0013), ale počas sledovacieho obdobia už pokles nebol významný. Manifestný astigmatizmus klesol z -4,89 ± 1,71 D na -1,64 ± 2,27 D (p < 0,0001) 6 mesiacov po operácii a na -2,26 ± 2,77 D (p = 0,0004), -2,88 ± 2,42 D (p = 0,0061), a -2,40 ± 1,19 D (p = 0,0045) v 7., 10. a 15. roku pooperačne. Topografický astigmatizmus klesol po 6 mesiacoch z 5,30 ± 2,05 D na 3,78 ± 2,03 D (p = 0,017), ale v 7., 10. a 15. roku po operácii už pokles nebol významný. Záver. Po 6 mesiacoch od operácie LASIK došlo v súbore pacientov k významnému zlepšeniu priemernej nekorigovanej zrakovej ostrosti, sférického ekvivalentu, topografického a manifestného astigmatizmu. Zlepšenie priemernej UCVA a MA bolo signifikantné aj v 15. roku sledovania.
