Mechanisms of bacterial resistance to aminoglycosides:genetic bases and test methods of resistance

Abstract:

Aminoglykosidická antibiotika jsou stejně jako tetracykliny řazeny do skupiny antibiotik, které inhibují syntézu proteinů navázáním na 30S podjednotku bakteriálního ribosomu. Jejich spektrum účinku je široké, působí na aerobní gramnegativní i grampositivní bakterie baktericidně. Jsou účinné především při infekci způsobené druhy bakterií čeledi Enterobacteriaceae a dále rody Pseudomonas a Acinetobacter spp. Bakterie si však vůči různým typům antibiotik vybudovávájí různé mechanismy resistence. Daná antibiotika pak nejsou účinná. Nejvýznamnějším mechanismem resistence vůči aminoglykosidům je tvorba modifikujících enzymů: acetyltransferas, fosfotransferas a nukleotidyltransferas. Resistence vůči antibiotikům se stala celosvětovým problémem. Geny kódující enzymy se nachází na chromosomech i mobilních genetických elementech, které jsou příčinou jejich rychlého šíření mezi mikroorganismy. Byla vyvinuta řada metod, které detekují geny kódující resistenci. Mezi nejpoužívanější techniky patří PCR, hybridisace, gelová elektroforéza. Novější metodou je použití mikročipů, které jsou citlivější a rychlejší. Tato práce má za cíl charakterizovat aminoglykosidy, jejich působení a strukturu, dále popsat mechanismy resistence vůči aminoglykosidům i geny kódující resistenci a nakonec vymezit metody molekulární biologie, které nám mohou pomoci geny resistence detekovat.

Abstract:

Both aminoglycoside antibiotics and tetracyclines belong to the group of antibiotics which inhibits protein synthesis with binding to the 30S ribosomal subunit. Aminogylcosides are bactericidal and broad-spectrum antibiotics. They are particularly active against aerobic gram-negative and gram-positive bacteria. They work on infections caused by members from family Enterobacteriaceae and by genera Pseudomonas and Acinetobacter spp. Many bacteria have developed diverse resistance mechanisms against many different antibiotics. These antibiotics are then inactive when they are used. Production of aminoglycoside modification enzymes has become the most frequent resistance mechanism: acetyltransferases, phosphotransferases and nucleotidyltransferases. Drug resistance is worldwide problem. Genes encoding enzymes are placed on chromosomes as well as on mobile genetic elements, which are the cause of rapid dissemination of drug resistance genes between microorganisms. It was developed range of methods detecting resistance encoding genes. The most widely used are PCR, hybridisaton, gel electrophoresis. More recent methods are microarrays which are more sensitive and faster. The aim of this work is to describe aminoglycosides, their antibacterial spectrum and activity, after that define resistance mechanisms against aminoglycosides and resistance encoding genes and in the end determine Molecular biology methods, which detect resistance genes.
