The effect of physiotherapy on lumbar spine pain

Abstract:

Název: "Vliv fyzioterapie na bolesti bederní páteře" Úvod: Bolest bederní páteře je v dnešní době zcela běžná a jednou do roka se s bolestí setkává alespoň 40% pacientů. Přibývá negativně působících vlivů na pohybový systém a počet pacientů stoupá se současně se snižující věkovou hranicí. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky u pacientů, kteří přichází k terapii pro bolesti bederní páteře. Bolest zad je celoživotní problém, a proto je velmi důležitá kvalitní edukace pacientů v tom, jak si pomoci od bolestí páteře sami. Metodika: V bakalářské práci jsem zpracovala problematiku pacientky s bolestmi bederní páteře. Jako metodu první volby jsem použila McKenzie terapii, a to z důvodu akutního stavu, ve kterém pacientka přišla. Na základě získaných informací z vyplněného spisu dle MDT jsem stanovila diagnózu derangement unilaterální asymetrický pod koleno. V terapii jsem použila nejdříve extenční princip statický v lehu na břiše s vypodložením polštáři. Později dynamický extenční princip, který bolest v LDK snižoval. Podle krátkodobého fyzioterapeutického plánu jsem použila další metodiky jako baňkování a měkké techniky k uvolnění spazmů na lýtku a svalech Th-Lp, dále koncept vzpěrných cvičení podle Roswithy Brunkow, cvičení hlubokého stabilizačního systému, kineziotaping, Dornovu metodu a spirální stabilizaci MUDr. Smíška. Provedla jsem korekci špatných pohybových stereotypů a pro správnou korekci sedu jsem doporučila bederní váleček. Výsledky: Výstupní kineziologický rozbor prokázal úspěšnost terapie. U pacientky došlo ke zlepšení rozsahu pohybu do extenze, ke snížení subjektivního vnímání bolesti v bederní páteři. Na začátku vyšetření bylo VAS 6-7/10 a na konci terapie 1/10. Celkové zlepšení stavu vnímala i pacientka. Parestezie v lýtku jsem odstranila. Osvědčilo se cvičení na neurofyziologickém podkladě podle Roswithy Brunkow, ACT a metoda spirální stabilizace. Rozdílné zatížení mezi PDK a LDK se výrazně upravilo. Diskuse: Porovnání dosažených výsledků se studiemi uvedenými v teoretické části. Závěr: V porovnání s počátečním stavem pacientky došlo k výraznému zlepšení, ale i tak se nezabránilo v operativním řešení výhřezu ploténky na neurochirurgii Fakultní nemocnice.

Theme: Beneficial effect of physitheraphy on lumbar spine´s pain. Introduction: Nowadays lumbar spine´s pain is very common problem and once a year at least forty percent of people suffer from this. The amount of harmful effects on locomotor system is incerasing and the number of patients rises simultaneously with decreasing age limits. The aim of the thesis: The aim of my bachelor thesis is beeing instrumental in solving problems of patients who come to use the theraphy because of lumbar spine´s pain. Back pain is a lifetime problem, that is why good patient´s education in helping themselevs with backache is very important. Methodology: In my bachelor thesis is studied the problem of Loir back pain. The first method I used was the Mc Kenzie theraphy. The reason of using it was urgent state of health of a woman when she came to me. I established the diagnosis unilateral asymmetric derangement under the knee based on information from filled file MDT. During my therapy first I used the static extension principle lying on her belly with pillow support. Later I used dynamic extension principe, which made the pain in area of left lower limb. According to short time physiotheraphy plan I used next methods like cupping and soft tissue techniques for releasing spasms on the calf and muscles Th-Lp. Next I used concept struting exercises based on Roswithy Brunkow, ACT, deep stabilization system exercises, Dorn´s method of kineziotaping and Doctor Smišek´s spiral stabilization. I made correction of bad locomotor stereotypes and suggested lumbar roller for good correction of sitting. Results: Final kinesiologic analysis demonstrated the success of therapy. The range of locomotor to extension was better and her subjective perception of pain in THE lumbar spine was lower. At the beginig of the examination the VAS was in a rate of 6-7/10 and at the final treatment was 1/10. The patient felt better too. I removed calf paresthesias. Practising exercise based on Roswithy Brunkow and spiral stabilization made very good effects. The difference between the rihgt lower limb and the left lower limb loading was very adjusted. Discussion: Comparison of the results obtained with those of the theoretical part. Conclusion: In diffence between enatrance and final condition there was well-marked improvement, but on the other side it did not prevent her from an operation of slipped disc at the neurosurgery department.
