Forensic analysis of bone fracture caused by blunt force trauma

Abstract:

Předkládaná disertační práce je kompilací tří vědeckých publikací, jež popisují experimentální výzkum inspirovaný reálnými forenzními případy Laboratoře forenzní antropologie a odontologie (Labanof), kde byl celý výzkum uskutečněn. První část práce se věnuje zkoumání charakteristik zlomenin na mikroskopické úrovni s cílem najít specifické znaky, jež by bylo možné využít pro rozlišení zlomenin vzniklých perimortem od zlomenin vzniklých postmortem. Průběh lomné linie a její vztah ke kostní mikrostruktuře byl zkoumán na tenkých nedekalcifikovaných řezech recentních a historických kostí s pomocí světelného mikroskopu. Výsledky ukázaly, že lom postupoval u recentních i historických vzorků skrze osteon spíše přímo, ve většině případů tak při svém průběhu nerespektoval mikrostrukturu osteonu. Tento jev může být způsoben vysokou rychlostí úderu, který zlomeniny způsobil. Druhá část výzkumu tak byla zaměřena na charakteristiky zlomenin způsobených různými mechanismy o vysoké nebo naopak nízké rychlosti. Morfologie zlomenin vzniklých na zvířecích kostech explozí, kompresí a tupou silou byly studovány na makroskopické i mikroskopické úrovni. Účelem bylo ověření existence specifických znaků, které by bylo možné využít pro diferenciální diagnostiku mechanismů způsobujících studované trauma. Makroskopické pozorování přineslo velmi variabilní výsledky. Žádná z charakteristik zlomenin nebyla úzce svázána s konkrétním mechanismem vzniku této zlomeniny. Mikroskopické zkoumání ukázalo, že lom postupuje skrze kost rozdílně při vysoké a nízké rychlosti útoku. Analýza průběhu lomné linie může poskytnout důležité informace o mechanismu vzniku zlomeniny. Poslední část výzkumu testovala možnosti detekce kovových částeček na povrchu kosti po úderu tupým předmětem. Tři různé kovové tyče byly použity k vytvoření zlomenin na zvířecích kostech. Přítomnost kovových residuí byla následně zkoumána s využitím SEM-EDS. Kovové částečky odpovídající použitému nástroji byly nalezeny v okolí okrajů zlomeniny u všech použitých tyčí. Kovový předmět, který trauma způsobil, může být úspěšně zjištěn pomocí SEM-EDS analýzy.

Abstract:

The present thesis is a compilation of three scientific papers describing experimental research that was inspired by real forensic cases solved at the Laboratory of Forensic Anthropology and Odontology (Labanof) where a whole research was performed. The first part is dedicated to the observation of fracture characteristics at a microscopic level in order to find out the specific markers that can be used for distinguishing between perimortem and postmortem fractures. The fracture lines and their relation to bone microstructure were studied on thin undecalcified samples from fresh and dry human bone using light microscopy. The results showed that the crack propagates rather indiscriminately through the osteon then respects its microstructure in fresh as well as in dry samples, which could be caused by the high velocity of impact that produced the fractures. The next part of the research was therefore focused on the examination of fractures caused by different mechanisms of high and low energy. The morphology of fractures produced on porcine bones by explosion, compression and blunt trauma were studied macroscopically and microscopically in order to verify the existence of specific markers that would be useful for differential diagnosis of trauma mechanism. The macroscopic observation of fracture morphology gave very variable results, any fracture characteristic successfully indicating the manner of fracture origin was not found. The microscopic examination showed that the fracture propagates through bone microstructure differently in high compared to low energy impact. The analysis of the course of the fracture line could therefore give sufficient information about the fracture mechanism. The last experiment aimed at testing the possibility to detect metal residues on a bone surface after blunt force impact. Three different metal bars were applied on bovine bones and the presence of metal particles was analysed using SEM-EDS. The metal residues coinciding with where the bar impacted were seen near the fracture borders in all used instruments. The metal tool producing the trauma could be successfully determined using SEM-EDS analysis.
