Effect of hippotherapy in hemiparetic form of cerebral palsy

Abstract:

Cíl: Cílem bakalářské práce je ozřejmit postavení hipoterapie v pohybové léčbě a v komplexní péči o dítě a zpracovat konkrétní kazuistiku probanda s DMO, u kterého byl vybrán jako fyzioterapeutický přístup hipoterapie. Metodika: U vybraného probanda s hemiparetickou formou DMO jsem provedla dne 13. 6. 2014 vstupní kineziologické vyšetření s následným navržením krátkodobého terapeutického plánu. Na základě tohoto vyšetření jsem aplikovala týdenní intenzivní hipoterapii. Při výběru koně, tempa, terénu a vhodných hipoterapeutických pomůcek jsem vycházela z teoretických poznatků o hipoterapii a z kineziologického vyšetření probanda. Dne 3. 10. 2014 jsem provedla kontrolní kineziologické vyšetření, na jehož základě jsem navrhla druhou část krátkodobého terapeutického plánu a aplikovala jsem opět týdenní intenzivní hipoterapii. Dne 10. 10. 2014 jsem provedla výstupní kineziologické vyšetření a doporučila dlouhodobý terapeutický plán a následnou péči. Pro zhodnocení efektu terapie jsem použila specifické testy, které jsem zvolila konkrétně pro tuto kazuistiku. Výsledky: Výstupní vyšetření ukázalo oproti vyšetření vstupnímu zlepšení probandova stavu a to jak po motorické, tak po komunikační stránce. Po 2 týdnech intenzivní hipoterapie můžeme pozorovat zlepšení držení těla, dechového stereotypu, větší relaxaci LHK a LDK, větší rozsahy pohybů v kloubech, lepší funkci LHK, schopnost fyziologického vzorce lezení a schopnost samostatného stoje a chůze. Do stoje se již proband dostává bez patologické dopomoci HKK přes pozici šermíře či medvěda. V chůzi využívá disociace pánve a je schopen přenosu váhy. Zmizel náznak poruchy tělového schématu. V řeči již není opožděn, avšak projevila se horší výslovnost a lehká dysfázie, ke které měl předpoklady.

Abstract:

Objective: The objective of this thesis is to show the position of hipotherapy in the motion and complex treatment of child with cerebral palsy and to compose concrete case study with patient who has cerebral palsy and who was chosen to be treated with hippotherapy. Methodology: On a date 13. 6. 2014 I made entry exploration and I suggested short term therapeutic plan with chosen patient with hemiparetic form of cerebral palsy. On the basis of the entry exploration the one week-long intensive hippotherapy was applied. While choosing the horse, pace, ground and proper hippotherapeutic tools I relied on theoretical knowledge of hippotherapy and on entry exploration. On the date 3. 10. 2014 I made control exploration. On its basis I suggested second part of short term therapeutic plan and I applied another week-long intensive hippotherapy. On the date 10. 10. 2014 I made final exploration and I suggested long term therapeutic plan with follow-up treatment. To evaluate the effect of therapy I used specific tests which I choose specifically for this case report. Results: The difference between entry and final explorations showed the improvement in both, in motion and in communication abilities of chosen patient. After two weeks of intensive hippotherapy there is visible improvement of postural habits, breathing stereotype. Also improvement in ability to relax upper and lower left extremities, bigger movement in joints, better function of left upper extremity, ability of physiologic pattern of crawling and ability of independent stand and also ability of independent walk. To the standing position the patient gets without pathologic patterns, without helping himself with his hands. While walking, he can use decomposition of his pelvis and he can work with his centreof gravity. There is not anymore posture disability. The speech is not delayed anymore, but dysphasia and problems with pronunciation started, but those problems were prognosticated.
