Open and laparoscopic liver resection: Impact of obesity on treatment results of patients with primary and secondary liver tumors

Abstract:

Obezita je společensky i medicínsky velmi závažné onemocnění, které ve 21. století dosahuje pandemických rozměrů. Současný celosvětový nárůst prevalence obezity se významně podílí na nemocnosti a úmrtnosti civilizačních chorob. Obezita je spojena zejména se zvýšeným výskytem kardiovaskulárních, metabolických, respiračních, gastrointestinálních i některých nádorových onemocnění, včetně nádorů jater. Teoretická část práce je zaměřena na vysvětlení pojmu obezity, anatomie a fyziologie jater, nádorových onemocnění jater, diagnostických metod a jaterní chirurgie. Cílem mé diplomové práce je zjistit vliv obezity na výsledky léčby u pacientů s primárními a sekundárními nádory jater léčených dvěma různými operačními přístupy otevřené operace ve srovnání s operací laparoskopickou. Retrospektivní analýza prospektivně sbíraných dat jaterních resekcí byla provedena u 197 pacientů na Chirurgické klinice ve Fakultní nemocnici Ostrava za období od ledna 2007 do září 2013. Pacienti byly rozděleni do dvou skupin na základě definice obezity dle Světové zdravotnické organizace na obézní pacienty (25,4 %) a neobézní pacienty (74.6 %). Sledovaná data zahrnují demografickou charakteristiku, operační a pooperační parametry. Z výsledků vyplývá, že obezita nemá statisticky významný vliv na výsledky léčby pacientů s primárními a sekundárními nádory jater. Chirurgie jater je tedy bezpečná a proveditelná u obézních i neobézních pacientů.

Abstract:

Obesity represents one of the medical and social problems reaching the level of pandemic in the 21st century. Increase in the prevalence of obesity is significantly associated with a higher morbidity and mortality. Likewise, it is related to a higher incidence of cardiovascular, metabolic, respiratory, gastrointestinal diseases and cancer including primary liver tumours and metastases. The theoretical part of my thesis presents the fundamental definition of obesity, liver anatomy and physiology, liver oncology, diagnostic tools, and liver surgery. The aim of practical part was focused on the impact of obesity on the clinical outcome in patients with primary and secondary tumours treated by open or laparoscopic surgery. Retrospective analysis of prospectively collected data of liver resections (open/laparoscopic) in 197 patients, who were treated at Department of Surgery (Faculty hospital Ostrava) from January/2007 to September/2013, was done. Patients were divided into two groups according to the actual WHO obesity definition. Obese group involved 25,4% and non-obese group 74,6% of patients. Available demographic characteristics and surgical and post-surgical parameters are presented. Results did not show any significant differences between the obese and non-obese patients with primary or secondary liver tumours treated by open or laparoscopic surgery. Liver surgery regardless the open or laparoscopic approach is safe and feasible both in obese and non-obese patients.
