Presepsin, the use of early inflammatory biomarker in the diagnosis of invasive candidiasis in critically ill patients

Abstract:

Do této práce bylo zahrnuto celkem 166 pacientů, z toho 59 mělo v hemokultuře prokázanou kandidémii a 107 pacientů bakteriémii z celkového počtu 776 epizod. Vzorky byly získány během let 2017-2019 z nemocnic spádové oblasti Zdravotního ústavu se sídlem v Ostravě. U všech pacientů byly v séru měřeny časné zánětlivé biomarkery, leukocyty, C-reaktivní protein (CRP), presepsin (PSEP), prokalcitonin (PCT) a panfungální antigen (BDG), poté jsme jednotlivé biomarkery porovnávali z hlediska diagnostiky kandidové sepse s kontrolní skupinou bakteriémií. Mezi skupinami byl zjištěn statisticky významný rozdíl u CRP a BDG. Největší využitelnost v predikci kandidové sepse byla prokázána v případě BDG (AUC = 0,9964) pro hodnotu cut-off 200 pg/ml. Na skupině 17 pacientů léčených antimykotiky (echinokandin) bylo zjištěno, že je PSEP dobrým ukazatelem pro monitoring efektivity terapie. Zjištěný rozdíl mezi poklesem hladin BDG a PSEP v séru byl statisticky významnýčtrnáctý den léčby. Oba biomarkery, PSEP i BDG jsou přínosné pro současnou diagnostiku, management a terapii invazivní kandidózy a predikci kandidémie.

Abstract:

A total of 166 patients were included in this thesis, of which 59 had candemia in their hemoculture and 107 patients had bacteremia from the full number of 776 episodes. The samples were obtained within the years 2017-2019 from the hospital areas of the health institute based in Ostrava. The serum in all patients was measured for inflammatory biomarkers, leukocytes, C-reactive protein (CRP), presepsin (PSEP), procalcitonin (PCT) and panfungal antigen (BDG), afterwards we compared biomarkers from the perspective of diagnosis with a control group of bacteremia. A statisticaly significant difference was found between CRP and BDG. BDG (AUC = 0.9964) for the value of the cut-off 200 pg / ml. In the evaluation of 17 patients treated with antifungals (echinocandin), PSEP was found to be a good indicator for monitoring the effectiveness of therapy. The difference found between the decrease in serum BDG and PSEP levels was statistically significant on day 14 of treatment. Both biomarkers, PSEP and BDG, are useful for the current diagnosis, management and therapy of invasive candidiasis and the prediction of candidiasis.
