The rehabilitative intervention of an elderly patient in ischemic stroke outcome in a clinical presentation of vascular Parkinsonism: a clinical case

Abstract:

Parkinson's disease is a pathology affecting the nervous system, with causes that are still unknown, which affects the quality of movement causing tremors, rigidity and bradykinesia (Cartabellotta, 2018). The purpose of this thesis is to make known to the reader the general anatomy of the central nervous system, the basal ganglia and the physiology of the dopaminergic system, to further understand how this it is affected by the pathology. The symptomatology of Parkinson's disease will be further investigated, in the specific, how it affects posture and gait and how, although total remission of the pathology is not possible, its symptoms can be contained through drug therapies (Leta, 2019). The management of the motor symptoms of the pathology is a very important topic, for the maintenance of adequate mobility and a correct balance during the journey, decisive factors for an autonomous development of daily life activities. Finally, the case of R.P. was investigated, diagnosed with Parkinson's disease, followed by an ischemic stroke that led to a hemiparesis, the patient was followed in the Stella del Mattino at the Orizzonte Speranza Foundation - a no-profit organization during the internship. His physical condition and his management of walking will be compared despite the different pathologies, at the entrance and at the end of the rehabilitation course.

Abstract:

Il morbo di Parkinson ? una patologia a carico del sistema nervoso, dalle cause ancora sconosciute, che influisce sulla qualit? del movimento provocando tremori, rigidit? e bradicinesia (Carabellotta, 2018). Lo scopo di questa tesi ? portare a conoscenza del lettore l'anatomia generale del sistema nervoso centrale, dei gangli della base e la fisiologia del sistema dopaminergico, per approfondire poi in che modo viene colpito dalla patologia. Si andr? successivamente ad approfondire la sintomatologia del morbo di Parkinson, come influisce su postura e deambulazione ed in che modo, pur non essendo possibile una totale remissione della patologia, si possano contenerne i sintomi attraverso terapie farmacologiche (Leta, 2019). La gestione dei sintomi motori della patologia ? un tema molto importante, per il mantenimento di un'adeguata mobilit? e un corretto equilibrio durante il cammino, fattori decisivi per uno svolgimento autonomo delle attivit? di vita quotidiane. ? stato infine approfondito il caso di R.P., a cui ? stato diagnosticato il morbo di Parkinson, seguito da un ictus ischemico che l'ha portato ad una emiparesi, il paziente ? stato seguito nella struttura Stella del Mattino presso la Fondazione Orizzonte Speranza - onlus nel periodo di tirocinio. Verranno valutate le sue condizioni fisiche e la sua gestione della deambulazione nonostante le diverse patologie, all'ingresso e alla fine del percorso riabilitativo.
