Audit investigation into nursing documentation completed

Abstract:

Cíl: Cílem bakalářské práce byl audit uzavřené ošetřovatelské dokumentace, zaměřený na ošetřovatelské překladové zprávy. Metodika: Předmětem auditu byl tištěný formulář ošetřovatelské překladové zprávy, podle něhož byla vytvořena kontrolní kritéria k auditu. Audit probíhal na dvou odděleních a bylo zkontrolováno 48 ošetřovatelských překladových zpráv. Výsledky: Auditní šetření ukázalo, že ze 48 ošetřovatelských zpráv bylo zcela správně sepsáno 9. Nejčastější chyby byly u vylučování, kde chybělo zapsat datum poslední stolice, u kontaktní osoby chybělo uvést, zda tato osoba byla o překladu informována, u kvality spánku nebylo označeno, zda pacient má či nemá problémy se spánkem, u bolesti chybělo označit intenzitu nebo lokalizaci bolesti, u soběstačnosti s pomocí nebyla uvedena činnost, ve které se dopomoc týká. Úspěšnost vyplnění ošetřovatelských překladových zpráv byla od 100 % do 79 %. Aby byly odstraněny tyto nedostatky, měl by management informovat zdravotnický personál o výsledcích auditu, provést nápravná opatření a po stanovené době provést auditní šetření, které by ukázalo, zda nápravná opatření byla účinná. Závěr: Ošetřovatelské překladové zprávy musí být sepsány pečlivě a pravdivě, jelikož je to prvotní dokument týkající se ošetřovatelské péče, se kterým se pracovníci jiných oddělení, jiných zdravotnických a sociálních zařízení nebo domácí péče setkávají. Abstract:

Goal: The aim of this thesis was the audit investigation of completed nursing documentation, based on transfer nursing reports. Methods: The audit concerned the printed form of transfer nursing reports, which created the screening criteria for the audit. The audit was conducted in two sections and 48 nursing documentations were examined. Results: The audit survey showed that only 9 from all 48 nursing transfer reports were correctly completed. The most frequent errors were in the secretion part, which lacked a recorded date of the last stool; at the section about the contact person, the part that indicated whether the person was informed about the transfer was missing; in the section about the quality of sleep was not marked whether or not the patient has sleep problems. In the section dealing with pain management, part of labels which showed intensity or localization of pain was missing. In the self-sufficiency with help section, there was no mention in which activities the help was needed. Successful completion of nursing transfer forms was in the range from 100% to 79%. To eliminate these shortcomings, management should inform medical personnel of the results of the audit, and implement corrective actions. After a specified time should be allowed to establish if these measures have been effective. Conclusion: Nursing transfer reports must be drafted carefully and truthfully because it is the primary document on nursing care with which staff of other departments, other health and social facilities or home care will use.
