Analysis and therapy of gait in people with Charcot-Marie-Tooth disease

Abstract:

Diplomová práce se zabývá chůzí u osob s chorobou Charcot-Marie-Tooth. Hlavním cílem práce je zhodnotit vliv dlouhodobé terapie pomocí systému zebris-Rehawalk? na vybrané parametry chůze u osob s touto chorobou. V teoretické části práce je popsán klinický obraz choroby a její klasifikace, dále teoretická část obsahuje obecný popis chůze a také charakteristiku patologických vzorů chůze u osob s chorobou Charcot-Marie-Tooth. Výzkum byl uskutečněn v prostorách Lékařské fakulty Ostravské univerzity. Zúčastnilo se jej 6 osob (54,3 let ? 10,1 let) s chorobou Charcot-Marie-Tooth. Každý z nich v průběhu šesti měsíců podstoupil celkem 16 terapií na dynamickém chodníku h/p/cosmos?Rehawalk? se softwarem zebris. K vstupnímu, kontrolnímu a výstupnímu měření byl využit také tento přístroj a dále pak šestnáctikanálový povrchový neinvazivní elektromyografický přístroj TeleMyo 2400T G2 se softwarem MR3 (NORAXON). Z výsledků studie je patrné, že dlouhodobá terapie má u těchto pacientů vliv na některé parametry chůze. Bylo prokázáno statisticky významné prodloužení kroku a také prodloužení trajektorie COP (center of pressure) ve fázi mezistoje. Vliv terapie na bioelektrické parametry na vybraných svalech dolních končetin se neprokázal.

Abstract:

The diploma thesis deals with gait in people with Charcot-Marie-Tooth disease. The main goal of this work is to evaluate the effect of long-term therapy using the zebris-Rehawalk? system on selected gait parameters in people with this disease. The theoretical part of the thesis describes the clinical picture of the disease and its classification. Moreover, it contains a general description of gait and also the characteristics of pathological patterns of gait in people with Charcot-Marie-Tooth disease. The research was carried out on the premises of the Faculty of Medicine of the University of Ostrava. It was attended by 6 people (54.3 years +- 10.1 years) with Charcot-Marie-Tooth disease. Each of them underwent a total of 16 therapies on a dynamic gaitway h/p/cosmos?Rehawalk? with zebris software over the course of six months. This device was also used for input, control and output measurements, as well as a sixteen-channel surface non-invasive electromyographic device TeleMyo 2400T G2 with MR3 software (NORAXON). The results of the study show that long-term therapy has an effect on some gait parameters in these patients. A statistically significant elongation of the step was demonstrated, as well as a prolongation of the trajectory COP (center of pressure) in the midstance phase. The effect of therapy on bioelectrical parameters on selected lower limb muscles was not demonstrated.
