Depression in the elderly with chronic heart failure

Abstract:

Cíl: Cílem diplomové práce bylo zjistit výskyt deprese u seniorů s chronickým selháním srdce, hospitalizovaných na kardiovaskulárním oddělení. Metodika: Výzkumné šetření probíhalo formou anonymního dotazníkového šetření. Pro získání dat byl využit standardizovaný dotazník GDS (Geriatrická škála deprese), se zaměřením na sociodemografické údaje a informace, týkající se onemocnění. Soubor tvořilo 103 respondentů s chronickým selháním srdce, hospitalizovaných na Kardiovaskulárním oddělení Fakultní nemocnice Ostrava, v období od února do listopadu, roku 2016. Výsledky: Při zjišťování a hodnocení výskytu deprese u seniorů s chronickým selháním srdce, byly statisticky významné výsledky nalezeny v závislost deprese a věku (p = 0,026) a v závislosti deprese a rodinného zázemí (p = 0,019). Ostatní výsledky nebyly statisticky významné. Nicméně, v souboru byla deprese nejvíce vyskytována u těchto faktorů: ženy; věk 75-84 let; NYHA III-IV; lehká závislost dle ADL (Index Barthelové); 1-2 hospitalizace; senioři žijící doma sami a senioři vyučení a se základním vzděláním. Závěr: Senioři s chronickým srdečním selháním, hospitalizovaní na kardiovaskulárních odděleních trpěli depresí. Deprese se vyskytovala v nadpoloviční většině sledovaného souboru, a to v různé míře. Je tedy žádoucí detekovat depresi dříve, nežli má negativní dopad na přidružené onemocnění pacienta. K tomu by mohla sloužit Geriatrická škála deprese (GDS), která by mohla být vedle nutričního screeningu, sociální screeningu, rizika pádu a rizika dekubitů, základním screeningovým nástrojem pro detekci deprese na standardních odděleních.

Abstract:

Aim: The aim of the diploma thesis was to find the occurrence of depression in the elderly with chronic heart failure, hospitalized in the cardiovascular department. Methodology: The research was conducted in the form of an anonymous questionnaire survey. A standardized GDS (Geriatric Depression Scale) questionnaire, focusing on socio-demographic and disease-related information, was used to obtain the data. The group consisted of 103 respondents with chronic heart failure, hospitalized at the Cardiovascular Unit of the University Hospital Ostrava, from February to November, 2016. Results: Determination and evaluation of depression in seniors with chronic heart failure revealed statistically significant results depending on depression and age (p = 0.026) and depression and family background (p = 0.019). The other results were not statistically significant. However, the group was the most common depression among these factors: women; Age 75-84; NYHA III-IV; Light dependence according to ADL (Barthel Index); 1-2 hospitalization; Seniors living alone and seniors teach and have basic education. Conclusion: Seniors with chronic heart failure, hospitalized in cardiovascular departments suffered from depression. Depression occurred in the overwhelming majority of the monitored group, to varying degrees. It is therefore desirable to detect depression before it has a negative impact on the associated disease of the patient. A Geriatric Depression Grid (GDS) could serve as a basic screening tool for depression detection in standard departments, alongside nutritional screening, social screening, fall risk and stroke risk.
