Rehabilitation program of a sports patient following osteosynthesis due to a displaced fracture of the fibular head: a clinical case

Abstract:

The knee joint is one of the main parts of the human body, joining the upper part of the leg, formed by the femur, with the lower part, consisting of tibia and fibula. Such articulation plays a fundamental role in the walking phase and therefore has the main task of ensuring mobility and stability, characteristics that at first glance may seem contradictory. The knee can be easily exposed to trauma and pathologies, sometimes complicated, to the point of compromising daily activities. Knee problems can cause not only on-site pain, but a series of complications on the whole complex of the lower joints of the body, as they will create rewards to cope with the primary deficit. The fractures involving this joint are varied: from ligamentous lesions, such as those of LCA, to bony ones, such as fractures of the tibial plate, menial fractures, proximal fibula fractures, and many others. As mentioned above, following a traumatic event, there may be fractures, closure or exposure, dislocations and crushing trauma that are often complicated by severe nerve, venous and arterial injuries. The knee area is surrounded by numerous nerves, which play a key role mainly in the movements of the tibio-tarsica joint. Among these, plays an important role, the sciatic external popliteal nerve (SPE) or common peroneal nerve which is primarily the nerve of the lower arm most involved as a result of knee distortions, fractures of the third distal of the femur, tibial plate fractures and decomposed fibula head fractures. In the most serious cases, surgery is necessary, while if we are faced with less important trauma, rehabilitation physiotherapy plays a fundamental role. The aim of physiotherapy is to speed up healing, return to normal daily activities and, in the case of sports patients, the resumption of training.

L'articolazione del ginocchio ? una delle principali del corpo umano, mette in giunzione la parte superiore della gamba, formata dal femore, con la parte inferiore, costituita da tibia e perone. Tale articolazione svolge un ruolo fondamentale nella fase di deambulazione e ha, di conseguenza, il compito principale di assicurare mobilit? e stabilt?, caratteristiche che a primo impatto possono sembrare in contraddizione tra loro. Il ginocchio pu? essere facilmente esposto a traumi e patologie, a volte complicate, a tal punto di compromettere le attivit? quotidiane. Problematiche riguardanti il ginocchio, possono provocare non solo dolore in sede, ma una serie di complicazioni su tutto il complesso delle articolazioni basse del corpo, in quanto si andranno a creare compensi per far fronte al deficit primario. Le fratture che coinvolgono questa articolazione sono varie: dalle lesioni legamentose, come quelle dell'LCA, a quelle ossee, come fratture del piatto tibiale, fratture meniscali, fratture prossimali del perone, e tante altre. Come gi? detto in precedenza, in seguito ad un episodio traumatico, ci possono essere fratture, chiuse o esposte, lussazioni e traumi da schiacciamento che sono spesso complicati da gravi lesioni a nervi, vasi venosi e arteriosi. L'area del ginocchio ? circondata da numerosi nervi, i quali ricoprono un ruolo fondamentale principalmente nei movimenti dell'articolazione tibio-tarsica. Tra questi, ricopre un ruolo rilevante, il nervo sciatico popliteo esterno (SPE) o nervo peroneo comune che ? in primo luogo il nervo dell'arto inferiore pi? coinvolto in seguito a distorsioni del ginocchio, fratture del terzo distale del femore, fratture del piatto tibiale e fratture scomposte della testa del perone. Nei casi pi? gravi, ? necessario l'intervento chirurgico, mentre se ci troviamo di fronte a traumi meno importanti, la riabilitazione fisioterapica ricopre un ruolo fondamentale. Lo scopo della fisioterapia ? quello di velocizzare la guarigione, il ritorno alla normalit? quotidiana e, nel caso di pazienti sportivi, la ripresa degli allenamenti.
