Prenatal toxoplasmosis

Abstract:

Kolem toxoplazmózy v souvislosti s těhotnými ženami koluje mnoho mýtů. Cílem této práce bylo v Národní referenční laboratoři (NRL) pro toxoplazmózu zjistit především v kolika případech došlo ke kongenitální toxoplazmóze za rok 2010. Toxoplazmóza u imunokompetentních osob způsobuje celoživotní latentní infekci. Závažné onemocnění však způsobuje v případě přenosu z matky na plod během gravidity nebo při reaktivaci latentní infekce u imunodeficientních osob. Z celkového počtu 1918 pacientů bylo 136 těhotných žen (ve věku 19 až 44 let) a 41 dětí (novorozenců do jednoho roku života). Zaznamenala jsem pozitivní nález u 26 pacientů, negativní nález u 74 pacientů a výskyt anamnestických protilátek ukazujících na latentní infekci u 77 pacientů. Z celkového počtu 41 dětí byl pozitivní nález pouze u dvou novorozenců, tedy v 0,05 % případů. Výsledky získané zpracováním dat v NRL pro toxoplazmózu a diskuze s odbornými články mě vedly k zhodnocení, že toxoplazmózy se v České republice není třeba obávat a to při dodržení určitých opatření. Zároveň se jeví jako zásadní zlepšení informovanosti mezi laickou veřejností např. vhodným informačním plakátem v ordinaci gynekologa. Pro včasnou léčbu je potřeba zavčas zjistit, kdy se žena nakazila a proto navrhuji zavést opakované sérologické vyšetření na toxoplazmózu tak, jako už je zvykem ve Francii, Německu a Rakousku.

Abstract:

There are lots of myths about toxoplasmosis in connection with pregnant women. The main purpose of the work was to find in the National Reference Laboratory (NRL) for toxoplasmosis out the number of cases of congenital toxoplasmosis appeared in year 2010. Toxoplasmosis in immunocompetent people causes lifelong latent infection. However, the toxoplasmosis causes serious illness in case of transmission from mother to fetus during pregnancy or during reactivation of latent infection in immunodeficient individuals. From the total number of the 1918 patients there were 136 pregnant women (aged 19 to 44 years) and 41 children (newborn up to one year old). I have noticed a positive finding in 26 patients, negative findings in 74 patients and the incidence of anamnestic antibodies indicating latent infection in 77 patients. From the total number of 44 children there were positive finding just in two cases of neonates, that means in 0,05%. Results obtained by processing data in the NRL for toxoplasmosis and discussion with research articles led me to evaluate that toxoplasmosis in the Czech Republic is nothing to be worry about in compliance with certain measures. It also appears to be essential to improve awareness among the general public, for example to hang appropriate information posters in the gynecologist's consulting room. For early treatment is needed to determine when the woman became infected, and therefore I propose to implement repeated serological screening for toxoplasmosis as it is a custom in France, Germany and Austria.
