Abstract:

Bakalářská práce shrnuje problematiku propouštění geriatrických pacientů z nemocnice. Zabývá se postoji a názory zdravotníků na kontinuitu péče o seniory po propuštění z lůžkového oddělení do domácího ošetřování. Zároveň na tuto oblast nahlíží z pohledu samotných seniorů a tyto názory srovnává mezi sebou. V praktické části autorka informuje o průběhu průzkumného šetření, které bylo realizováno mezi geriatrickými pacienty a zdravotníky Masarykovy nemocnice v Ústí nad Labem v období od října 2007 do ledna 2008. Výsledky šetření ukázaly, že proces propouštění s důrazem na sociální rizika v praxi funguje ne zcela uspokojivým způsobem. Ke zkvalitnění procesu propouštění by bylo třeba rozvinout především individualizovanou péči a systematičtěji zapojit do práce se staršími pacienty zatím nedoceňované ergoterapeuty. Hlavní přínos této práce spočívá především v možnosti využití získaných údajů jako výchozího bodu pro hlubší zkoumání dané problematiky. Výsledky mohou být také využity v rámci sestavení návrhu standardu propouštění z nemocnic, který by zajišťoval co nejvyšší kontinuitu péče.

Abstract:

This dissertation summarizes the aspects of discharging geriatric patients from hospital. It concerns with attitudes and opinions of health professionals on the continuity of health care provided to the elderly people after having been discharged from stay-in hospitals to the home care. The dissertation also presents the views of the elderly patients themselves and compares the both perspectives. In the empirical part the author informs about a research that had been realized from October 2007 to January 2008 among geriatric patients and health professionals in Masaryk`s Hospital in Ústí nad Labem. The results of the research have shown that the process of discharging, especially when it comes to its social risks, is still not yet running satisfactorily enough. The further development of the individualized health care should be implemented in order to increase the quality of discharging process. More systematic involvement of occupational therapists might also be of a great benefit. The predominant asset of this dissertation lies particularly in providing useful data for further research of the matter. The results may also be useful when proposing a health care standard for discharging from hospital. This standard would enable the highest health care continuity possible.
