Rehabilitation of patients with vestibular problems

Abstract:

Úvod: Z širšího tématu vestibulární problematiky jsem se ve své bakalářské práci zaměřil na terapii pacienta s tinnitem neboli ušním šelestem. Jedná se o sluchový vjem, který lidské ucho vnímá bez přítomnosti vnější příčiny. Podle odborníků se číslo pacientů s tímto symptomem pohybuje celosvětově kolem 10 %. Příčina je často nejasná a může zahrnovat jak trauma způsobené hlukem, tak nádory v oblasti středního ucha, aterosklerózu, Meniérovu chorobu, potíže s krční páteří, případně dlouhotrvající stres. Náplní praktické části bakalářské práce je spolupráce s pacientem, trpícím pravostranným, subjektivním tinnitem. Za pomoci fyzioterapie, rehabilitačních a relaxačních jsem se pokusil ovlivnit probandův fyzický stav a zjistit tak, zda má fyzioterapie vliv na zmírnění intenzity ušního šelestu. Metody: První z metod, které jsem využil k úvodnímu náběru informací o stavu pacientky, je rozhovor. Ten jsem použil při seznámení pacientky s tématem mé práce a odebrání anamnézy. Další metodou je pozorování, které jsem využil při kineziologickém rozboru. V rámci zkoumání hybnosti krční páteře a ramenních kloubů pacientky, jsem využil měření pomocí goniometru. Během terapie jsem využil vhodné rehabilitační metody: měkké techniky, zahrnující ošetření kůže a podkoží pomocí Kiblerovy řasy, protažení fascií, akupresuru, postizometrickou relaxaci (PIR) a míčkování, mobilizace, zaměřující se na zlepšení kloubní hry (joint play) v zablokovaných segmentech krční páteře a relaxační techniky, z nichž jsem využil autogenní trénink. Výsledky: Během terapie se podařilo pozitivně ovlivnit významným způsobem jak pacientčin fyzický a psychický stav, tak ušní šelest. Jedná se především o subjektivní vnímání intenzity tinnitu a frekvenci, s jakou se symptom objevuje. Ušní šelest pacientku již tolik neobtěžuje a jsou i dny, kdy jej nepociťuje vůbec. Z hlediska fyzického, se podařilo napravit svalové dysbalance v oblasti šíjové a odstranit blokády krční páteře. Celkové zlepšení mělo velký vliv i na stav psychický. Závěr: Díky spolupráci s pacientkou a studiu odborné literatury jsem obohatil své poznatky o mnoho informací, týkajících se ušního šelestu a jeho léčby pomocí fyzioterapie. Jelikož se tinnitus nepodařilo odstranit úplně, zůstává i nadále problémem, který je potřeba přijmout. Podařilo se ale splnit cíl práce - pozitivně ovlivnit pacientčin tinnitus a významně jí tak ulehčit každodenní život.

Abstract:

Introduction: From the broader topic of vestibular problems, in my bachelor thesis I focused on the therapy of patient with tinnitus. It´s an auditory perception that is perceived by the human ear without the presence of external cause, which is bothering more and more people nowadays. Experts say, the number of patients with this symptom is around 10 % worldwide. The cause is often unclear and may include noise trauma, middle ear tumours, atherosclerosis, Meniere´s disease, cervical spine problems, prolonged stress and many more. The practical part of the thesis is focused on cooperation with patient, who suffers from right-sided, subjective tinnitus. With the aid of physiotherapy, rehabilitation and relaxation I tried to influence patient´s physical condition and to find out, whether physiotherapy has an effect on reducing the intensity of tinnitus. Methods: The first method which I used to initial collection of information about the patient´s condition, is an interview. I used it to acquiant the patient with the topic of my thesis and to take a case history. Another method is observation, which I used in kinesiological analysis. Then, I examined the ability to move of patient´s cervical spine and shoulder joints with use of goniometer. This is the third method I used - measurement. During the therapy I applied appropriate rehabilitation methods: soft techniques (treatment of skin and subcutaneous tissue, fascia stretching, acupressure, postisometric relaxation and balling), mobilization (improving joint play in blocked segments of the cervical spine) and relaxation (autogenic training). Results: After a series of ten therapies, both the patient´s physical, psychological condition and tinnitus were positively influenced. Subjective perception of symptom was significantly reduced. Tinnitus doesn´t bother the patient as often as before therapies and sometimes she doesn´t perceive the symptom at all. From a physical point of view, we managed to correct dysbalance associated with the cervical spine and neck muscles. The overall improvement had a signifiant influence on the psychological condition of patient. Conclusion: Thanks to the cooperation with the patient and the study of specialized literature, I have enriched my knowledge with a lot of information concerning tinnitus and it´s treatment by physiotherapy. Altough tinnitus hasn´t been completely removed, it has been positively affected and significantly facilitate patient´s daily life.
