The Issue of Migraine in Terms of Occupation Medical Care

Abstract:

Vstupujete do Dantova pekla migreniků. To je citát z knihy Migrénový mozek americké autorky Carolyn Bernsteinové. [1] Tato neuroložka je sama migreničkou a věnuje se celý svůj život pomoci takto stejně nemocným lidem. Onemocnění známé už od dob faraónů si získalo větší pozornost zdravotníků až v posledních letech. Jedná se o onemocnění vrozené, nevyléčitelné a většinou velmi bolestivé. Jeho průběh lze modifikovat v omezené míře prevencí, relativně úspěšně profylaxí a léčením akutních migrénových záchvatů. Etiologie není zcela objasněná. Rozšíření tohoto onemocnění v česku zatím není dokumentováno, stejně tak jako náklady na profylaxi a léčení. Cílem práce je formou epidemiologických studií přispět k poznatkům o souvislostech projevů migrény a efektivity prevence a léčení tak, aby získané údaje byly využitelné v ZPP, protože nejčastěji se udává začátek migrenózních potíží na startu pracovní kariéry ve věku kolem 20 let. Zkoumanými soubory bylo 323 studentů LF Ostravské Univerzity a 122 pacientů soukromé neurologické ambulance v Ostravě. Konkrétně se jednalo o 30 migreniků a dále o 92 pacientů s vertebrogenním syndromem, kteří tvořili kontrolní skupinu. U studentek se závažné bolesti hlavy vyskytovaly u 17,0 % respondentek. U migreniček se srovnatelně časté bolesti hlavy vyskytovaly u 35,0 % žen a u pacientek s vertebrogenním syndromem u 12,0 % žen. Z výsledků je zřejmé, že bolesti hlavy pravidelně omezují lidskou práceschopnost. Ze studentek 65,1 % uvedlo, že občas musí kvůli bolesti hlavy ulehnout a 11,3 % často. 68,2 % migreniček uvedlo, že mívá tak intenzivní bolesti hlavy, že si musí lehnout občas a 13,6 % ulehne často. Bolesti hlavy jsou velmi frekventované a zatím málo sledované a ošetřované symptomy či diagnózy. U trvale léčených pacientů neurologické ambulance se vyskytovala diagnóza migrény jen u 2 %, z celkového počtu 2500 trvale sledovaných pacientů. Účinnost profylaxe migrény jsme ovšem vyhodnotit nemohli, protože v souboru migreniček bylo jen 13,6 % pacientek ošetřovaných tímto způsobem.

[1] She is a neurologist and sufferers itself by migraine and she helps migraine patients. This disease known since the time of the Pharaohs has gained greater attention of medics only in recent years. It is a congenital, incurable and usually very painful disease. Its course can be modified by a limited extent prevention, relatively successful prophylaxis and treatment of acute migraine attacks. The etiology is not entirely clear. The extension of this disease has not yet been documented in the Czech Republic, as well as the cost of prophylaxis and treatment. The aim of this work is to contribute by epidemiological studies to knowledge of migraine symptoms connections and effectiveness of prevention and treatment in such a way, that the data will be useful in preventive medical care, because most often the initiation of migraine troubles begins at the start of work career around the age of 20 years. Examined files were 323 students of Medicine Faculty of Ostrava University and 122 patients of private neurological ambulance in Ostrava. There were 30 migraine patients and 92 patients with vertebrogenic syndrome, who created a cohort group. Severe headache occurred in 17 % of female students, in 35.0 % of migraine females and at 12.0 % of women with vertebrogenic syndrome. The results indicate that the headaches regularly restrict human employability. 65.1 % of female students said that they sometimes have to lie down because of headache and 11.3 % of them have to lie down often. 68.2 % of migraine women reported that their headaches are so intense that they must lie down sometimes and 13.6 % of them have to lie down frequently. Headaches are very frequent and not yet widely observed and treated symptoms or diagnosis. Only 2 % of the total number of 2,500 continuously monitored patients of the neurological ambulance are treated with migraine diagnosis. The prophylaxis effectiveness of migraine, however, we could not evaluate, because of only 13.6 % of female patients treated this way.
